纳米碳对甲状腺手术中甲状旁腺的保护作用

摘    要:目的:探讨纳米碳用于甲状腺手术中对甲状旁腺的保护作用。方法将72例患者随机分为两组各36例,对照组按甲状腺手术常规进行;观察组在甲状腺手术中,于甲状腺上下取2~5个点,在真被膜下方注入0.1~0.2 mL纳米碳混悬液,待甲状腺和周围淋巴组织染色后,完整游离甲状腺外科包膜,保护未染色组织,再常规行甲状腺手术。结果观察组术后发生低钙血症2例(5.6%),对照组6例(16.7%),两组差异有统计学意义(χ^2=13.87,P<0.05);观察组术后发生甲状旁腺功能减退1例(2.8%),对照组为5例(13.9%),两组差异有统计学意义(χ^2=14.53,P<0.05);观察组病理切片证实无误切甲状旁腺,对照组误切甲状旁腺7例(19.4%),两组差异有统计学意义(χ^2=127.54,P<0.05)。结论在甲状腺手术中使用纳米碳,可帮助鉴别淋巴结及甲状旁腺,从而降低甲状旁腺的损伤率。

Abstract:Objective To discuss the role of naso-carbon in the protection of parathyroid during surgery. Methods 72 patients were randomly divided into the two groups,Injecting 0.1-0.2mL nano-carbon in the thyroid during a thyroid surgery.When the thyroid and surrounding lymph tissue were stained,then the thyroid surgery should be continued,the non-stained tissue should be protected.Results There were 6 cases with hypocalcemia in the con-trol group and 2 cases in the observation group(χ^2=13.87,P〈0.05).5 cases with PTH decrease in the control group and 1 case in the observation group(χ^2=14.53,P〈0.05).7 parathyroid glands were examined in the control group,while none in the observation group(χ^2=127.54,P〈0.05).Conclusion Using naso-carbon during the thy-roid surgery,parathyroid gland cannot be stained black.The parathyroid gland can be easily identified and preserved.
