上颌窦神经鞘瘤的诊断和治疗

目的 探讨上颌窦神经鞘瘤的临床特点、诊断和治疗.方法 回顾性分析1987～2006年收治的4例上颌窦神经鞘瘤病人的临床资料.结果 本组上颌窦神经鞘瘤病人男3例,女1例,年龄16～24岁;镜下病理学表现为束状型1例,网状型3例;全部病例均手术切除,术后病理确诊,3例术后随访4～20年未见复发,1例随访3年,未见复发,后失访.结论 上颌窦神经鞘瘤多为单发,一般包膜完整,易于完整切除,但由于症状、体征、影像学表现缺乏特异性,应注意与其他上颌窦疾病相鉴别,确诊靠病理学检查.手术完整切除是治疗该病的主要方法.

Diagnosis and therapy of neurilemmoma of sinus maxillaris

Objective To explore the clinical features,diagnosis and therapy of neurilemmoma of sinus maxillaris. Methods retrospectively analyzed clinical data of 4 cases of neurilemmoma of sinus maxillaries who were treated between 1987 and 2006 in our hospital. Results Of the 4 patients,3 were male and 1 female,aged between 16 and 24. Histopathologic examination showed 3 cases were Antoni A, and 1 Antoni B under microscope. All patients were successfully treated by operation and diagnosed by the pathological method. 3 cases were followed up for 4 to 20 years with no tumor recurrence,1 case were followed up for 3 years with no recurrence ,but lost contact later on. Conclusion Neurilemmoma of sinus maxillaris commonly arise as solitary lesion with intact capsule,and is easily misdiagnosed due to its weak specificity of clinical features. Final diagnosis depends on pathological examination. Complete resection of tumor lesions is the main therapy for this kind of disease.
