摘    要:目的 探讨256层螺旋CT(SCT)与冠状动脉造影诊断冠状动脉病变的价值.方法 选择冠状动脉硬化性心脏病患者46例,均行冠状动脉造影及SCT检查.比较两种检查方法对冠状动脉病变的检出情况.结果 (1)所有SCT及冠状动脉造影均取得满意的图像,在对冠状动脉狭窄及闭塞的诊断中,两种检查方法间差异无统计学意义(P>0.05).(2)依照冠状动脉造影的标准,SCT对冠状动脉狭窄诊断的敏感性为93.0% (132/142),特异性为91.6% (142/155),阳性预测率为91.0%( 132/145),阴性预测率为93.4%( 142/152).结论 SCT有很高的时间分辨率、空间分辨率,对冠状动脉病变的显示具有较高的敏感性、特异性、阳性预测率和阴性预测率.

Abstract:Objective To explore the clinical value of 256-slice CT and coronary angiography in the diagnosis of coronary artery disease.Methods 46 patients with coronary heart disease,who underwent coronary angiography and 256-slice spiral CT were selected.Compare two methods for the detection of coronary artery disease.Results (1)All 256-slice CT and coronary angiography achieved a satisfactory image,in the diagnosis of coronary artery stenosis and occlusion,the two methods had no statistically significant difference(P>0.05).(2)according to standard coronary angiography,256-slice spiral CT for the diagnosis of coronary artery stenosis sensitivity was 93.0%(132/142),specificity was 91.6%(142/155),positive predictive rate was 91.0%(132/145),negative predictive rate was 93.4%(142/152).Conclusion 256-slice spiral CT has a high time resolution,spatial resolution,the display of the coronary artery disease has high sensitivity,specificity,positive predictive value and negative predictive value.
