结核分枝杆菌对氧氟沙星、左氧氟沙星体外耐药监测

摘    要:目的了解菌阳肺结核患者对氟喹诺酮药物的耐药现状,为临床制定标准化方案提供参考依据。方法选取2008年3月至2009年3月期间天津市海河医院门诊及住院菌阳肺结核病患者119例,对其结核分枝杆菌(MTB)临床分离株做氧氟沙星(OFLX),左氧氟沙星(LVFX)药物敏感性试验。结果初治患者耐OFLX 10例(14.5%),耐LVFX 3例(4.3%);复治患者耐OFLX 30例(60.0%),耐LVFX 22例(44.0%);敏感菌耐OFLX 16例(19.0%),耐LVFX 8例(9.5%),耐药菌耐OFLX 24例(68.6%),耐LVFX 17例(48.6%);单耐药、多耐药、耐多药、严重耐多药耐OFLX分别为2例(40.0%)、10例(66.7%)、7例(77.8%)、5例(83.3%),耐LVFX分别为2例(40.0%)、6例(40.0%)、5例(55.6%)、4例(66.7%);氟喹诺酮用药时间≤2周、2周至1月、〉1-3月、〉3-6月、〉6月,耐OFLX分别为2例(33.3%)、4例(36.4%)、7例(50.0%)、9例(100%)、5例(100%);耐LVFX分别为1例(16.7%)、2例(18.2%)、6例(42.9%)、6例(66.7%)、5例(100%)。结论复治结核病对氟喹诺酮的耐药率较高,耐药结核菌对氟喹诺酮耐药率也明显高于敏感菌,而严重耐多药及耐多药菌株耐药率明显高于其他耐药类型结核病。试管内耐药浓度与耐药率呈负相关。制定标准方案时LVFX优于OFLX。
