基于SHA2011的云南省经常性卫生费用核算结果分析

目的:对2018年云南省经常性卫生费用进行核算,分析云南省经常性卫生费用的筹资现状、存在的问题及其原因,为制定卫生费用政策提供依据。方法:运用卫生费用核算体系2011(SHA 2011),结合云南省实际情况,核算云南省2018年经常性卫生费用。结果:2018年云南省经常性卫生费用为1300.54亿元,占卫生总费用(THE)的78.61%,占云南省GDP的7.27%。公共筹资、家庭卫生支出和自愿筹资分别占62.91%、32.76%和4.34%。流向医院和基层医疗卫生机构的费用合计占73.87%,流向公共卫生机构费用占6.19%。费用的74.42%用于治疗服务,仅有9.73%用于预防服务。结论:2018年云南省经常性卫生费用总量较为合理,但居民看病就医经济负担仍较重,经常性卫生费用的机构配置待优化,重治疗,轻预防现象亟待转变,应采取针对性的政策措施加以解决或改善。

Analysis on the Accounting Results of Yunnan Current Health Expenditure Based on SHA 2011

Objective:To calculate the current health expenditure in Yunnan in 2018,analyze the current status,existing problems and causes of current health expenditure in Yunnan Province,and provide references for designing related policies on health.Methods:Combined with the actual situation in Yunnan,System of Health Accounts 2011(SHA 2011)was applied to estimate current health expenditure in Yunnan in 2018.Results:The current health care expenditure amounted to 130.054 billion yuan in 2018,accounting for 78.61%of the total health expenditure(THE)and 7.27%of GDP.Public financing scheme,voluntary funding scheme and household out—of-pocket payment accounted for 62.91%,32.76%and 4.34%.The total expenditure to hospitals and primary medical institutions accounted for 73.87%,and the expenditure to public health institutions accounted for 6.19%.74.42%of the cost was applied for treatment services,only 9.73%for prevention services.Conclusion:Total current health expenditure in Yunnan in2018 was reasonable.In 2018,the total current health expenditure in Yunnan province was relatively reasonable,but the economic burden of residents’medical treatment was still heavy.the institutional allocation of recurrent health care expenses needed to be optimized.Treatment is more important than preventionphenomenon needed to be changed,and targeted policy measures should be introduced to solve or alleviate these problems.
