70岁以上老年患者肠梗阻185例诊治分析

目的总结我院10年来治疗老年肠梗阻诊治的经验。方法回顾性分析10年间收治的185例老年肠梗阻资料。结果病因明确169例（91.6%）,其中机械性肠梗阻167例（90.9%）,原因不明16例（8.4%）;手术治疗91例（49%）,非手术治疗94例（51%）;总死亡率9.2%（17例）,手术治疗死亡4例（2.1%）。结论老年性肠梗阻病因以粘连性、肿瘤性为常见;高龄和并存病不是手术的禁忌;做好围手术期处理,把握好手术指征,选择合理的治疗方式是保证治疗成功的关键。

Review of the Management of Intestinal Obstruction of 185 Over-70 Years Patients

Objective To reveiew our esperience in treatment of senile patients with intestinal obstruction in the recent 10 yeats. Methods The data of 185 senile patients with intestinal obstruction were analyzed retrospectively, Results The obstructive factors in 159 cases （91. 6% ）were definite, including mechanical obstruction （ 167cases,90.9% ）, and there were 15 cases （ 8.4% ） with unknown causes. 91 cases （ 49% ） underwent surgical treatment and 94cases（51% ）subjected to nonsurgical treatment. Total mortality was 9.2% （17cases）. The opera- tive mortality was 2.1% （4 cases）. Conclusion The common factors of senile intestinal obstruction were adhesive- ness and tumor. The senium and comorbidities were not the contraindication of surgical treatment. Perioperative management, holding of the operative indications and the selection of teasonable procedures are the key to the suc- cessful treatment.
