全容积三维超声心动图定量评价正常人、冠心病患者的左心室收缩功能

摘    要:目的:探讨新技术全容积三维超声心动图(FV-3DE)定量检测正常人、冠心病患者左心室整体及各节段收缩功能的可行性与准确性. 方法:使用全容积三维超声成像技术,于心尖四腔切面采集16例正常人及18例冠心病患者左心室的"金字塔"三维数据库,使用容积及收缩功能分析软件,脱机测算左心室整体及各节段的容积及收缩功能,并与二维超声双平面Simpson法测量结果进行对比. 结果:无论在正常人及冠心病患者,FV-3DE均可准确获得收缩末期和舒张末期的左心室容积、射血分数,与二维超声双平面Simpson法结果高度一致;另外,FV-3DE数据脱机分析还可全面获得整个心动周期中左心室各壁各节段的局部容积及射血分数变化曲线. 结论:全容积三维超声心动图可以准确测量左心室容积,计算左心室整体收缩功能,并全面分析左心室各节段的收缩功能,为系统且无创的研究左心室功能开辟了新途径.

Abstract:AIM: To validate the feasibility and accuracy of full volume three- dimensional echocardiography (FV-3DE) in evaluating the global and regional contractile function of left ventricle in normal persons and patients with coronary artery diseases. METHODS: FV-3DE images were acquired in 16 normal subjects and 18 patients with coronary artery disease by a matrix array transducer, the global and regional volume and ejection fraction (EF) of left ventricle were measured by Tom-Tec 4D LV-analysis software. At the same time, the results of global and regional volume and EF were compared with those measured by Simpson's method. RESULTS: The global volume and EF of left ventricle could be measured with FV-3DE well and truly in the normal subjects and the patients. There were no statistically significant differences among the results of FV-3DE and Simpson's method. Meanwhile, detailed data about regional volume and contractile function of left ventricle could be get by FV-3DE. CONCLUSION: FV-3DE is a novel, noninvasive technique capable of detecting global and regional volume and contractile function of left ventricle.
