慢性乙型肝炎肝组织病理损伤与肝功能、HBeAg及血清HBV DNA的关系研究

目的探讨慢性乙型肝炎(CHB)患者肝组织炎症活动度、纤维化程度与患者年龄、肝功能、血清HBVDNA载量、HBeAg的关系。方法经肝穿活检病理诊断CHB 262例,分析患者肝组织炎症活动度、纤维化程度与患者年龄、肝穿活检前后2周ALB、GLB、TBIL、ALT峰值、血清HBVDNA载量、HBeAg之间关系。结果肝组织炎症活动度、纤维化程度与GLB、TBIL、ALT呈正相关(r=0.164,0.306,0.447;0.164,0.280,0.338;P<0.01),纤维化程度与炎症活动度呈明显正相关(r=0.772,P<0.01);HBeAg阳性CHB肝组织炎症活动度、纤维化程度与HBV DNA载量无相关(r=-0.125,-0.171,P>0.05);HBeAg阴性CHB肝组织炎症活动度、纤维化程度还与年龄、HBV DNA载量呈正相关(r=0.194,0.318;0.224,0.317;P<0.05~0.01),与ALB呈负相关(r=-0.324,-0.347;P<0.01)。结论慢性乙型肝炎肝组织病理损伤程度与肝功能损害呈正相关;肝组织纤维化程度与炎症活动度相关;血清HBV DNA载量可作为HBeAg阴性CH...
