第二跖骨复合组织瓣移植修复手部复合组织缺损

目的 介绍采用不同形式的第二跖骨复合组织瓣.修复手部复合组织缺损的手术方法.方法 2003年5月-2007年8月,对9例手部复合组织缺损的患者,分别采用包含第二跖骨、跖趾关节、跗内侧皮瓣、跗外侧皮瓣、跖背皮瓣或伸肌腱等多种组织的复合组织瓣修复.结果 9例复合组织瓣全部存活.术后随访时间为1～6个月,骨、伸肌腱移植愈合良好,移植修复的掌指关节活动范围为0°～60°,修复部位外形满意.结论 采用第二跖骨复合组织瓣修复手部复合组织缺损是一种良好方法,能一次性修复缺损的复合组织.

Transfer of composite tissue flap from the second metatarsal to repair complex tissue defects of the hand

Objective To introduce various configurations of composite tissue flap transfer from the second metatarsal for treatment of complex tissue defects of the hand.Methods From May 2003 to August 2007,9 cases of complex tissue defects of the hand were treated by the several types of composite tissue flap which included the second metatarsal bone,metatarsophalangeal joint,medial tarsal flap,lateral tarsal flap,dorsal digital flap and extensor tendons.Results All the composite flaps survived.Follow-up period ranged from 1 month to 6 months.Bone union and tendon healing were excellent.The reconstructed metacarpophalangeal joint had 0° to 60° range of motion.The contour of the repaired site was satisfactory.Conclusion Composite tissue flap transfer from the second metatarsal is a good option for repairing complex tissue defects of the hand.It provides good blood supply and can achieve one-stage repair of all the missing tissues.
