慢性肾衰竭患者头孢菌素脑病28例临床分析

目的:探讨慢性肾衰竭患者头孢菌素类脑病的发病机制和临床特点、治疗方法。方法:对我科1998年1月～2009年6月共28例慢性肾衰竭患者使用头孢菌素类药物后出现的神经精神症状、治疗进行回顾性分析。结果:精神、神经症状表现多样。所有患者均无神经定位体征,CT检查无病理性改变,停药后症状消失。结论:慢性肾衰竭患者头孢菌素脑病的发病机制考虑与其肾功能不全导致排泄率低有关,其临床表现多种多样,应及早停药。患者多在2～6 d内缓解。

Clinical analysis of cephalosporin encephalopathy in 28 cases with chronic renal failure

Objective To explore the pathogenesis,clinical characteristics and treatment of cephalosporin encephalopathy in patients with chronic renal failure.Method The neuropsychiatric symptoms and treatment after cephalosporin drugs in 28 cases with chronic renal failure from January 1998 to June 2009 were analyzed retrospectively.Results Neuropsychiatric symptoms had different clinical manifestations.All of the patients had no neurological signs and positioning,CT showed no pathological changes,the symptoms disappeared after withdrawal.Conclusion The pathogenesis of cephalosporin encephalopathy in patients with chronic renal failure is related to the low excretion rates resulting from renal insufficiency,its clinical manifestations varied,should be early withdrawed.The patients will recover in 2～6 d.
