亚裔儿童ABCB1基因C3435T多态性对患急性淋巴细胞白血病危险性的相关研究

目的通过meta分析准确评估ABCB1基因C3435T多态性对儿童患急性淋巴细胞白血病(ALL)危险性的影响。方法在Embase、PubMed、Elsevier、CNKI、Wan Fang Data中检索关于ABCB1 C3435T多态性与儿童ALL风险之间关联的相关文献。95%置信区间和优势比用来评估两者之间的关联性,并对结果的稳定性和偏倚性做出评估。结果纳入的11篇文献含有对照组1681例,儿童ALL组2755例。隐性(TT vs CC+CT)模型、共显性(TT vs CC)模型中的ABCB1 C3435T多态性与ALL呈明显相关性(TT vs CT+CC:OR=1.54,95%CI=1.08~2.40,P=0.01;TT vs CC:OR=1.45,95%CI=1.06~2.18,P=0.03)。按人群种族进行亚组分析显示在高加索儿童中两者之间不存在显著的相关性(P>0.05);而在亚洲儿童中隐性与共显性模型均显著关联(TT vs CT+CC:OR=2.24,95%CI=1.16~4.48,P=0.01;TT与CC:OR=2.05,95%CI=1.08~3.92,P=0.02)。敏感性分析该研究的meta-analysis相对稳定。Begg's test和Egger's test结果排除了出版偏倚的可能性。结论meta分析表明亚裔儿童ABCB1基因C3435T多态性与ALL风险增加相关。

Meta-analysis of the Association between ABCB1 (MDR1) Gene C3435T Polymorphism and Risk of Childhood Acute Lymphoblastic Leukemia (ALL)

Objective To accurately assess the impact of ABCB1(MDR1)gene C3435T polymorphism on the risk of childhood acute lymphoblastic leukemia(ALL)by meta-analysis.Methods PubMed,Elsevier,CNKI and Wan Fang databases were used to retrieve the published literature on the association between ABCB1 C3435T polymorphism and children's ALL risk.The 95%confidence interval and odds ratio were used to assess the association between the two and to assess the stability and bias of the results.Results The included 11 literatures included 1681 control cases and 2,755 children in ALL group.The polymorphism of ABCB1 C3435T in recessive(TT vs CC+CT)model and codominant(TT vs CC)model was significantly correlated with ALL(TT vs CT+CC:OR=1.54,95%CI=1.08~2.40,P=0.01;TT vs CC:OR=1.45,95%CI=1.06~2.18,P=0.03).Subgroup analysis by population ethnicity showed no significant correlation between the 2 groups in Caucasian children(P>0.05).The recessive and codominant models were significantly correlated in Asian children(TT vs CT+CC:OR=2.24,95%CI=1.16~4.48,P=0.01;TT and CC:OR=2.05,95%CI=1.08~3.92,P=0.02).Sensitivity analysis of the meta-analysis of this study was relatively stable.Begg's test and Egger's test results exclude the possibility of publication bias.Conclusion Meta-analysis showed that the C3435T polymorphism of ABCB1 gene is associated with an increased risk of ALL in Asian children.
