二维及彩色多普勒超声诊断甲状腺癌的价值

目的探讨二维及彩色多普勒超声诊断甲状腺癌的价值。方法将100例甲状腺癌患者设为观察组,并选择同时间段治疗的80例甲状腺瘤患者作为对照组,2组患者均得到手术及临床病理证实。所有患者均接受二维及彩色多普勒超声诊断。对2组患者的超声诊断符合率、血流丰富程度、动脉收缩期峰值速度及阻力指数进行对比分析。结果经影像学检查结果显示,观察组患者确诊86例,诊断符合率为86.0%,其中10例患者误诊为甲状腺瘤,误诊率为10.0%,4例患者漏诊,漏诊率为4.0%。根据血流丰富程度看,2组二维及彩色多普勒超声表现差异均具有统计学意义(P<0.05)。观察组动脉收缩期峰值速度、阻力指数明显高于对照组,观察组周边无声晕、微钙化灶、低回声等影像表现率明显高于对照组,2组差异具有统计学意义(P<0.05)。结论二维及彩色多普勒超声诊断甲状腺癌具有较高的应用价值,可以帮助临床区分甲状腺瘤,为临床治疗提供参考方案,值得在临床中推广应用。

The Value of the Two-dimensional and Color Doppler Ultrasound Sonography in Patients with Thyroid Carcinoma

Objective To explore the value of the two-dimensional and color doppler ultrasound sonography in patients with thyroid carcinoma.Methods 100 patients with hyperthyroidism were divided into observation group,and 80 patients with thyrophyma were divided into the control group from the same time.Both groups were confirmed by surgery and clinicopathology,and all patients received two-dimensional and color doppler ultrasound diagnosis.The diagnostic accordance rate,the degree of blood flow,the systolic peak velocity of the arterial and the resistance index of both groups were compared.Results In the observation group,86 patients were confirmed,the diagnostic accordance rate was 86.0%,10 patients were misdiagnosed as thyrophyma,the misdiagnosis rate was 10.0%,4 patients were missed diagnosis,the omission diagnostic rate was 4.0%.As the degree of blood flow with color doppler ultrasound,two-dimensional and color doppler ultrasound sonography of the 2 groups had statistical significance(P<0.05).The systolic peak velocity of the arterial and the resistance index of the observation group were significantly higher than those of the control group,the utrasonographic characteristics,microcalcification and hypoechoic of the observation group were significantly higher than those of the control group(P<0.05).Conclusion The two-dimensional and color doppler ultrasound sonography in patients with thyroid carcinoma has high application value,which can help to differentiate patients with thyrophyma,provides a reference program for clinical treatment,and is worth popularizing in clinical practice.
