神经外科术后颅内感染的手术室相关因素分析

目的 探索神经外科术后颅内感染的手术室相关因素,为预防术后颅内感染提供理论依据。方法 采用横断面研究法,收集山西省人民医院2018年4月至2018年6月神经外科手术患者658例为观察对象,采用自行设计的信息采集表进行信息采集。按术后是否感染进行分组,通过单因素分析和logistic回归分析筛选术后颅内感染的危险因素。结果 在658例患者中,剔除47例不符合标准的患者,纳入611例,其中发生感染50例,感染率8.18%;急诊手术、有植入物、手术间开门次数、手术间人数和无菌手套使用时间≥4 h是术后颅内感染的危险因素。结论 严格把握人工植入物的使用指征,控制手术间人数和手术间开门次数,要求手术台上医护人员在手术持续时间≥4 h时更换无菌手套是降低术后颅内感染发生率的重要因素。同时对于急诊手术患者要严格控制手术指征,做好术前准备。

Relevant factors of intracranial infection after neurosurgery in operation room

Objective To explore the factors related to intracranial infection after neurosurgery. Methods We investigated 658 patients who received neurosurgery in a Shanxi Provincial People"s Hospital from April 2018 to June 2018. The clinical data was collected by using the self-designed information collection form. The patients were divided into two groups according to the intracranial infection. The risk factors of postoperative intracranial infection were analyzed. Results Among the 611 patients who were admitted to the group with surgery, the incidence of infection was 8.18%. Emergency surgery, implants, the times of opening operation room doors, the number of people in operation room and the length of aseptic gloves using were the risk factors of postoperative intracranial infection. Conclusions It is helpful to reduce the incidence of postoperative intracranial infection by strictly grasping the indication of artificial implants, controlling the number of populations in operating rooms, the times of opening doors and requiring medical staff to replace aseptic gloves on the operating table for more than 4 hours. At the same time, emergency surgery patients should strictly judge the surgical indicators.
