2型糖尿病合并冠心病患者血清内脂素水平的测定及影响因素

摘    要:目的探讨血清内脂素(visfatin)在2型糖尿病(T2DM)及其合并冠心病(CHD)患者中的表达情况及其影响因素。方法用酶联免疫吸附方法(ELISA)检测单纯T2DM组(n=30)、T2DM合并CHD组(n=34)及正常对照组(n=30)的血清内脂素水平和胰岛素(FINS)含量,用日立全自动生化分析仪测定空腹血糖(FPG)、血脂,计算所有观察对象的体质量指数(BMI)、腰臀比(WHR)。结果单纯T2DM组、T2DM合并CHD组内脂素水平均高于正常对照组(P<0.01),T2DM合并CHD组显著高于单纯T2DM组(P<0.01);内脂素水平与胰岛素抵抗指数(Homa IR)、WHR具有显著相关性(P<0.01)。冠脉病变支数与内脂素表达呈显著正相关(P<0.01)。结论T2DM合并CHD患者血清内脂素表达与Homa IR、WHR及冠脉病变支数存在密切的相关性,在T2 DM及合并CHD的发生发展中起着重要作用。

Abstract:Objective To investigate the level of plasma visfatin and its influencing factors in patients with type 2 diabetes melli-tus(T2DM) complicated with coronary heart disease (CHD). Methods The levels of visfatin and fasting insulin (FINS) in patient with T2DM, T2DM complicated with CHD and normal controls were measured by ELISA, and fasting plasma glucose (FPG) was measured by an automatic biochemistry analyzer. Body height and weight were measured to calculate body mass index(BMI) and waist to hip ratio( WHR) . Results The levels of serum visfatin in the two T2DM groups were higher than that in the normal control group ( P < 0.01) . The level of visfatin in the group of T2DM complicated with CHD was higher than that in the group of T2 DM ( P < 0.01) . Visfatin was positively correlated with Homa-IR and WHR ( P < 0.01) . The visfatin level was positively correlated with the numbers of involved coronary arteries( P < 0.01). Conclusions Visfatin plays an important role in the occurrence and development of T2DM and T2DM complicated with CHD.
