解毒通腑汤治疗毒蛇咬伤肢体肿胀的临床研究

目的 观察中药解毒通腑汤对毒蛇咬伤致肢体肿胀患者的临床疗效.方法 将68例毒蛇咬伤患者随机分为治疗组34例、对照组34例.对照组采用中西医结合常规的毒蛇咬伤治疗方案,治疗组在对照组治疗方案的基础上,予中药解毒通腑汤加减口服.观察2组患者咬伤后第2天及第6天的肢体肿胀程度,5-羟色胺水平的变化,促炎细胞因子白介素-6（IL-6）、肿瘤坏死因子-α（TNF-α）与抗炎细胞因子白介素-10（IL-10）水平的变化.结果 肢体肿胀程度2组在咬伤后第2天比较,差异无统计学意义（P＞0.05）,治疗组咬伤后第6天较本组咬伤后第2天降低（P＜0.01）,对照组咬伤后第6天较本组咬伤后第2天降低（P＜0.05）,治疗组前后差值与对照组比较差异有统计学意义（P＜0.05）,治疗组优于对照组.5-羟色胺水平2组在咬伤后第2天比较,差异无统计学意义（P＞0.05）,2组咬伤后第6天均较本组咬伤后第2天降低（P＜0.01）,治疗组前后差值与对照组比较差异有统计学意义（P＜0.01）,治疗组优于对照组.IL-6水平2组在咬伤后第2天比较,差异无统计学意义（P＞0.05）,治疗组咬伤后第6天较本组咬伤后第2天降低（P＜0.01）,对照组咬伤后第6天较本组咬伤后第2天降低（P＜0.05）,治疗组前后差值与对照组比较差异有统计学意义（P＜0.05）,治疗组优于对照组.TNF-α水平2组在咬伤后第2天比较,差异无统计学意义（P＞0.05）,治疗组咬伤后第6天较本组咬伤后第2天降低（P＜0.01）,对照组咬伤后第6天较本组咬伤后第2天降低（P＜0.05）,治疗组前后差值与对照组比较差异有统计学意义（P＜0.05）,治疗组优于对照组.IL-10水平2组在咬伤后第2天比较,差异无统计学意义（P＞0.05）,治疗组咬伤后第6天较本组咬伤后第2天升高（P＜0.01）,对照组咬伤后第6天较本组咬伤后第2天升高（P＜0.05）,治疗组前后差值与对照组比较差?

Clinical study of snakebite induced limb swelling with Jiedu Tongfu Tang

Objective To observe the curative effect of Jiedu Tongfu Tang in treating limbs swelling patients bitten by snakes. Methods Sixty-eight limbs swelling patients bitten by snakes were randomly divided into treatment group （n=34）and control group （n=34）. The control group was given conventional therapy with Chinese and western medicine, while the treatment group was treated with Jiedu Tongfu Tang and the conventional therapy. The limbs swelling, concentration of 5-hydroxytryptamine, IL-6, TNF-α, and IL- 10 were detected. Results The limb swelling showed no significant differences （P〉0.05）on day 2, and limb swelling of treatment group improved significantly （P〈0.01） on day 6, while the control group showed the same（P〈0.05）. Compared to the control group, the treatment group showed significant differences （P〈0.05）, and the treatment group was superior to the control group. Concentration of 5-HT, IL-6, TNF-α, and IL- 10 had no significant differences （P〉0.05） between the two groups on day 2. Compared to day 2, significant differences were shown in terms of 5-HT（P〈0.01）, IL-6（P〈0.01）,TNF-α（P〈0.05）, and IL-10（P〈0.01） on day 6. Compared to the control group, the treatment group showed significant difference（P〈0.01） in 5-HT and IL-10, and significant difference in IL-6 and TNF-α （P〈0.05） . Conclusion Jiedu Tongfu Tang is effective to limb swelling caused by snakebite.
