三亚市实施消灭脊髓灰质炎的策略与效果分析

目的：分析三亚市消灭脊髓灰质炎（以下简称脊灰）不同阶段的免疫策略与效果，为消灭及控制其它疫苗相关疾病提供经验，方法：收集不同阶段所采取的免疫策略，分析实施后产生的效果。结果：不同免疫阶段所产生的效果不同，消灭脊灰阶段免疫策略产生的效果最显。脊灰疫苗常规免疫接种率持续保持在85％以上，22轮次强化免疫接种率均达95％以上，人群抗体在1：400高免疫策略阶段脊灰年发率呈连续下降趋势，监测系统监测资料证实三亚市1993年4月后已连续8年无脊灰野病毒感染病例发生，实现全市无脊灰状态，消灭脊灰以来减少家庭经济负担1，533万－8，390万元，创造效益1，234万－6，669万元，结论：三亚市通过实施消灭脊灰免疫策略，已达到阻断脊灰野病毒的循环，在产生了显的经济效益同时也产生了巨大的社会效益，免疫策略为控制其它疫苗相关疾病提供了宝贵经验。

Analysis of the strategy and effectiveness of eliminating poliomyelitis In Sanya City

ve Tocnsolidae the achievement in eliminating poliomyelitis and provide the experience for controlling other diseases in Sanya city. Methods . The strategies employed in the control of poliomyelitis at various control and surveillance stages were gathered and the results gained in the emplementation of the control strategies were analysed. Results The rate of immunization has been increasing year by year. The routine immunization rate has been maintained at more than 85% from 1988.Twenty two rounds of intensifying immunization were carried out with a immuinzation rate of over 95%. The geometrical mean titer(GMT) of antibody against poliomyelitis in population was above 1:400 from 1995 to 2000. Annual incidence of poliomyelitis decreased year after year along with increase of immunization rate. Since 1994 no case infected with wild strain of polio vinus was found in this city. The rate of AFP case was more thanl/100000, and all other survillance index accord with the of government and WHO, since there was no reporting of the case, family economical burden was reduce by 15,330, 000 to 83,900,000 Yan, and create a great social economical benefit of 12,340,000 to 66,690,000 Yan. Conclusion By putting into practice the immunological strategy, the goal of eliminating poliomyelitis has been achieved, and it brought about a great benefit, and also provided a valuable experience for controlling other diseases.
