肝外胆管癌不同术式生存分析

目的:分析肝外胆管癌患者的预后因素.方法:多因素Cox模型分析预后因素.结果:手术方式和术后生存期的关系最为密切,其次是病理类型、术后并发症,建立了肝外胆管癌术后生存危险状态函数.探查术的1,3,5年生存率分别为11.48%,0.06%,0.003%;外引流术分别为30%,16.12%,0.31%,切除术分别为69.23%,28.10%,17.04%.结论:手术切除是治疗肝外胆管癌患者的最佳方案.

Analysis of prognostic factors of patients with extrahepatic bile duct carcinoma

Objective: To analyze prognostic factors of patients with extrahepatic bile duct carcinoma. Methods: Multi variate Cox model was used to analyze prognostic factors. Results: The relation between operation type and postoperative survival period was t he most close and postoperative survival period was obviously lengthened for pat ients with resection. Pathological type and postoperative complication were also proved to be correlated with postoperative survival period and mathematic funct ion was created to evaluate postoperative survival risk state of extrahepatic bi le duct carcinoma. Based on this, survival rates of 1,3, and 5 years after diffe rent operation types were compared. The survival rates of 1,3, and 5 years after exploration were 11.48%,0.06%,0.003%, respectively. The rates after extra-d rainage were 30%,16.12%,0.31%, and the rates were 69.23%,28.10%,17.04% for resection operation. Conclusion: Operation resection was opti mal scheme to treat patients with extrahepatic bile duct carcinoma.
