呼出气一氧化氮测定在老年AECOPD诊治中的作用

目的探讨呼出气一氧化氮(Fe NO)测定在老年慢性阻塞性肺疾病(COPD)诊治中的作用。方法对我院76例老年COPD急性发作期(AECOPD)患者进行Fe NO检测,并与同期收治的38例非呼吸系统疾病老年患者进行比较。结果治疗前Fe NO在COPD组(32.5±11.4)ppb]显著高于对照组(18.6±6.1)ppb](P0.01);COPD组Fe NO在出院时(25.1±7.8)ppb]较治疗前明显下降(P0.01),但仍显著高于对照组(P0.01)。以27 ppb为COPD患者治疗前Fe NO测定值的"切点",将76例COPD患者分为低Fe NO组31例(40.8%),高Fe NO组45例(59.2%),但两组之间症状轻重差异无显著性(P0.05)。结论 Fe NO在老年AECOPD患者中明显增高,Fe NO的高低不能作为COPD症状轻重的指标,但可指导治疗方案的选择。

Value of fractional exhaled nitric oxide in elderly patients with chronic obstructive pulmonary disease

Objective To investigate the value of fractional exhaled nitric oxide （ FeNO） in the diagnosis and treatment of elderly patients with chronic obstructive pulmonary disease （ COPD） . Methods 76 elderly patients with AECOPD were given FeNO test, and another 38 elderly patients without respiratory diseases were taken as the control group. Results Before the treatment, the level of FeNO was significantly higher in the COPD group （32. 5 ±11. 4） ppb] than in the control group （18. 6 ±6. 1） ppb] （P〈0. 01）. After the treatment, the level of FeNO decreased in the COPD group （25. 1 ± 7. 8） ppb], but it was still significantly higher than that in the control group （P〈0. 01）. There was no significant difference between the high FeNO group and the low FeNO group （P〉0. 05）. Conclusion The level of FeNO in elderly patients with AECOPD is significantly higher, but it can not indicate the symptomatic severity.
