胰岛素抵抗危险因素的探讨

目的　研究 2型糖尿病胰岛素抵抗的危险因素及其内在变化关系。方法　对 83例初诊 2型糖尿病患者 正常体质量组 34例 ,超重和 (或 )肥胖组 4 9例 ]胰岛素抵抗的 8种危险因素 ,包括空腹血糖、空腹胰岛素、三酰甘油、胆固醇、高密度脂蛋白胆固醇、收缩压、舒张压和体质量指数 ,进行了测定 ,并与 32例正常对照组比较。结果　 2型糖尿病组空腹血糖、空腹胰岛素、三酰甘油、血压较对照组高 (P <0 .0 1～ 0 .0 5 ) ,高密度脂蛋白胆固醇较对照组低 ;2型糖尿病正常体质量组与超重和 (或 )肥胖组间血糖、血脂、血压比较差异无统计学意义 ,但超重和 (或 )肥胖组空腹胰岛素水平高于正常体质量组 (P <0 .0 1)。结论　 2型糖尿病一旦诊断 ,多存在一定程度的胰岛素抵抗 ,这种抵抗与体质量增加、三酰甘油升高有关 ,且易形成高血压 ,直接影响糖尿病多种慢性并发症及其预后

Exploration into insulin resistance risk factors

Objective To investigate the inner connection between type 2 diabetes and insulin sensitivity resistance risk factors. Methods Eighty-three patients with type 2 diabetic in initial diagnosis (34 normal body mass and 49 over mass/obesity) and 32 normal controls were evaluated by 8 various risk factors (fasting glucose,fasting insulin, triglyceride, cholesterol, high density lipoprotein cholesterol, systolic pressure, diastolic pressure and body mass index). Results The levels of fasting glucose, fasting insulin,triglyceride, and blood pressure in type 2 diabetic patients are much higher than those in the control group (P< 0.01- 0.05 respectively).The level of high density lipoprotein cholesterol in type 2 diabetic patients is much lower than that in the control group. The levels of blood glucose, blood lipid and blood pressure in the normal mass group are not different from those in the over mass/obesity group, but the level of fasting insulin in the normal mass group is much lower than that in the over mass/obesity group (P< 0.01). Conclusion Once type 2 diabetes are diagnosed, insulin resistance exists in some degree. Insulin resistance is mostly connected with body mass growing, triglyceride rising, and hypertension easily develops, effecting directly on chronic complications and prognosis of type 2 diabetic patients.
