彩色多普勒超声应用于乳腺占位性病变诊断的临床分析

摘    要:目的探讨彩色多普勒超声在乳腺占位性病变诊断及鉴别诊断巾的价值。方法选择乳腺占位性病变患者100例,术前采刚彩色多普勒超声检查,观察其二维声像图及多普勒血流冈特征,并与手术病理检查结果进行对比分析。,结果与病理检查结果相比较,彩色多普勒超声检查诊断良性占位性病变81例,恶性占位性病变19例;其良性占位性病变诊断符合率为97.46%(77/79),恶性占位性病变诊断符合率为90.47%(19/21)。恶性占位性病变形态多不规则,纵横比多≥1,病灶边缘多模糊不规则,多有后壁回声衰减,常伴有微钙化灶;良性占位性病变形态规则,纵横比多〈1,病灶边缘多清晰规则,多无后壁回声衰减及微钙化灶。恶性占位性病变内血流信号丰富程度Adler分级0~Ⅲ级分别为2,2、8、10例,良性占位性病变分别为65、10、4、0例,两者比较,P均〈0.05。恶性占位性病变收缩期峰值血流速度(PSV)为(17.05±9.12)cm/s、阻力指数(RI)为(0.91±0.12),良性占位性病变分别为(8.55±5.32)cm/s、(0.63±0.23),两者比较,P均〈0.05。结论色多普勒超l声对乳腺占位性病变良恶性质的鉴别具有重要价值。

Abstract:Objective To investigate the value of color Doppler ultrasound in breast diagnosis and differential diagnosis of space- occupying lesions. Methods 100 cases of patients with space-occupying lesions of the breast, preoperative color Doppler ultra- sound to observe its two-dimensional sonographie and Doppler flow characteristics, and with the surgical pathological results were analyzed. Results Compared with the results of the pathological examination, color Doppler ultrasound diagnosis of benign space- occupying lesions in 81 cases, 19 cases of malignant space-occupying lesions; Color Doppler ultrasound two cases of simple can- cer misdiagnosed as benign occupying lesions benign mass lesion diagnosis rate of 97.46% (77/79), malignant space occupying le- sion diagnosis rate of 90.47% (19/21). Malignant space occupying lesion morphology irregular aspect ratio muhi ≥ 1 lesion edge multi-fuzzy irregular, muhi-posterior echo attenuation, often accompanied by microcalcifications; benign space-occupying lesion morphology rules, aspect ratio 〈1 lesion edge clear rules, and more posterior echo attenuation and microcalcifications. Malignant space-occupying lesions change the richness of the blood flow signal Adler grading o-m were 2,2,8,10 patients, benign space-oc- cupying lesions were 65,10,4,0 cases, the comparison between the two, P〈0.05. Malignant space-occupying lesions peak systolic velocity (PSV) (17.05±9.12) cm / s, resistive index (RI) is 0.91 ± 0.12, benign space-occupying lesions (8.55±5.32) cm/ s,0.63± 0.23, respectively, P〈0.05. Conclusion Color Doppler ultrasound identification nf the breast occupying lesions of benign and ma- lignant nature of the value of China Post.
