显微镜直视下冷凝联合眼内光凝治疗锯齿缘离断性视网膜脱离

摘    要:目的:探讨应用显微镜直视下冷凝联合眼内光凝治疗锯齿缘离断性视网膜脱离的手术效果.方法:对锯齿缘离断性视网膜脱离35例35眼,借助显微镜直视下巩膜外冷凝联合眼内激光光凝视网膜裂孔,对手术疗效、冷凝手术时间、手术并发症等进行随访和评价.结果:术后随访6~36个月,一次手术治愈率91.4%(32/35),盲目率从术前74.2%(26/35)下降到术后6个月的34.3%(12/35).手术平均冷凝时间(2.0±1.1)min.术后发生高眼压5例,白内障6例,视网膜下出血2例.结论:显微镜直视下冷凝联合眼内光凝治疗锯齿缘离断性视网膜脱离具有创伤小,术后视力恢复良好,视网膜复位率高及并发症少等优点.

Abstract:Objective:To evaluate the value of cryotherapy combined with intraocular laser photocoagulation for treatment of ora serrata dialysis retinal detachment under operation microscope. Methods: The cryoretinopexy combined with intraocular laser photocoagulation under microscope were applied in 35 cases (35 eyes) with ora serrata dialysis. The operative effect, time of cryotherapy and complication were analyzed. Results: Follow-up was from 6 to 36 months, 32 of 35 cases (91.4%) achieved retinal reattachment after the initial surgery. As compared with preoperative blindness rate (74.2%), the postoperative blindness rate (34.3%) was significantly decreased ( P 〈0.01). The operative time of cryotherapy was 2.0± 1.1 minutes. There was no severe complication. The incidence of postoperative high intraocular pressure, cataract occurred and subretinal hemorrhage was 5, 6 and 3 eyes, respectively. Conclusion: The cryoretinopexy combined with intraocular laser photocoagulation under microscope applied in ora serrata dialysis that can simplify operational procedure, shorten surgical time and decrease complication.
