B超测量IUD放置位置对避孕效果的影响

摘    要:
采用前瞻性群组研究方法,对963例对象置器后立即B超测量IUD上缘至宫底外缘距离并随访其避孕效果。测得距离平均为14.24±2.02mm,IUD的脱落频数随距离的增加而增大,置器后18个月,距离≤14mm对象组的粗累积脱落率明显低于距离>14mm对象组,距离每增加1mm,其脱落的相对危险增加1.25倍,但距离因素对妊娠率和因症取出率无明显影响。结果显示,把IUD顶端置入距宫底外缘距离≤14mm的区域内,更可能减少脱落,降低脱落的相对危险,提高续存率。提示有可能在B超的纵切观测下为“宫底部位”作出相应的定量概念。

Abstract:
A cohort study was introduced to evaluate the effects of quality of insertion on the performance of two IUDs (Tcu -Mand Mcu375sl) .A total of 963women with distances from the upper end of the vertical arm of the IUD to the external uterine fundus (distance) were measured with two-dimensional ultrasound using abdominal sector immediately after insertion and then followed - up prospectively 0 The mean value of the distances was 14.24 ± 2.02mm and the gross cumulative expulsion rate for the group which their distances were <14mm was much lower than those the distances were >14mm. No relationship was found, however, between the distance measured and either gross cumulative pregnancy or removal rates after 18 months of use.Results from the study indicated that it will be more likely to reduce the life table expulsion rate and lower the relative risk of being expulsed as well as increasing the continuation rate if the from IUD top to uterine fundus was < 14mm. As to the quality control of the IUD insertion, it might be possible to quantify the concept of the fundus of u-terine using two-dimensional ultrasound.
