非血管化游离髂骨移植血供重建的动物实验研究

摘    要:
目的:研究非血管化游离髂骨移植血供重建的时间和规律,探讨血供和新骨生成的关系.方法:选用杂种犬6只,造成一侧下颌骨2.5 cm全层骨缺损,自体游离髂骨移植.术后1、2、4月摄X线片、行下牙槽动脉血管造影后处死动物,明胶墨汁颈总动脉灌注.标本作组织学观察,行微血管记数.结果:术后1月:移植骨大部分被吸收,网状骨小梁增生,骨间隙内大量微血管新生.术后2月:植骨区大部分为新生骨所占据,新生骨小梁粗大,新生血管数目增多.X线示移植区低密度影.术后4月:新生骨基本矿化成熟,新生血管数目下降.X线示移植区高密度影.6例血管造影均未见下牙槽动脉再通.结论:非血管化游离髂骨移植新生血管术后1月开始大量增生,2月时数目最多,4月时趋于正常水平.新骨的生长是以微血管为中心,以膜内成骨和软骨内成骨两种方式多生长点成骨.

Abstract:
Objective:To study the blood supply regularity of non-vascularized bone grafts and the relationship between blood supply and new bone formation. Method: One sred 2.5 cm long defects were made in six dogs' mandibles and repaired with non-vascularized iliac bone. respectively X ray, angiography and ink artery infusion were taken 1,2,4 months after operation and then animals were sacrificed for histological analysis and microvessle counting. Recult: In non-vascularized iliac bone grafts,new microvessels began to increased in one month and decreased in 4 months after operation. 1 month postoperatively Large parts of the graft were absorbed and new trabecular bone and new microvessels were formed. 2 months postoperatively Main parts of the grafted area were occupied by newly formed bone and microvessels. X ray showed lower density in this area. 4 months postoperatively: Mature bone were formed and the number of newly formed microvessles decreased. X ray showed higher density in grafted area. Angiography established no blood supply in arteria alveolaris inferior. Conclusion: New bone formation were around the newly formed microvessels and the ways of bone formation were membrane bone formation and enchondral bone formation.
