兰州市住院孕产妇不良妊娠结局现况研究

目的:了解兰州市住院孕产妇不良妊娠结局的发生现状,为有针对性地制定降低不良妊娠结局发生率的有效措施提供理论依据。方法:采用整群随机抽样的方法抽取兰州市3所医院2004年1月~2005年12月产科住院孕产妇病历共6 825份的相关信息,并对相关因素进行分析。结果:兰州市住院孕产妇不良妊娠结局的发生率为14.65%,各种不良妊娠结局的发生率依次为:自然流产(8.01%)、早产(1.98%)、死胎及死产(1.16%)、新生儿窒息(1.13%)、巨大儿(1.04%)、出生缺陷(0.76%)、葡萄胎(0.57%);不良妊娠结局在<20岁和>40岁年龄段为最高,分别为30.12%和29.99%;农民和无业者不良妊娠结局的发生率高,分别为20.17%和17.65%。结论:兰州市住院孕产妇不良妊娠结局的发生率偏高,<20岁和>40岁的育龄妇女是重点防治对象,应加强对农民和无业者等弱势群体的扶持,从而降低不良妊娠结局的发生率。

Study on the current conditions of adverse pregnancy outcomes of hospitalized pregnant women in Lanzhou city

Objective:Understanding the present situation of adverse pregnancy outcomes of hospitalized pregnant women in Lanzhou city,in order to provide a theoretical basis for developping a targeted and effective measures to reduce the incidence of adverse pregnancy outcomes.Methods:From January 2004 to December 2005,the relevant information of total 6 825 medical records of obstetric hospitalized pregnant women from three hospitals in Lanzhou city were collected by cluster random sampling method,and analyzed the correlation factor.Results:The incidence of adverse pregnancy outcomes of hospitalized pregnant women in Lanzhou city was 14.65%,each kind of adverse pregnancy outcomes incidence rate was in turn: natural abortion(8.01%),premature delivery(1.98%),dead foetus and dead birth(1.16%),neonatal asphyxia(1.13%),large for date infant(1.04%),birth defect(0.76%),hydatidiform mole(0.57%).The incidence of adverse pregnancy outcomes was the highest in pregnant women less than 20-year-old and more than 40-year-old,the incidence rate was 30.12% and 29.99%,respectively.Farmers and the unemployed had high incidence of adverse pregnancy outcomes,the incidence rate was 20.17% and 17.65%,respectively.Conclusion:The formation rate of adverse pregnancy outcomes in Lanzhou city is high,women less than 20-year-old and more than 40-year-old toare the point prevention and cure object,should strengthen should strengthen supports to the vulnerable groups,such as farmers and the unemployed,thereby reducing the incidence rate of adverse pregnancy outcomes.
