他克莫司对截肢创伤应激后大鼠肝肾组织的影响

摘    要:
目的观察大鼠截肢创伤应激后所致肝脏和肾脏的损伤及他克莫司(FK506)对肝肾可能的保护作用,为截肢术后引起的远隔器官损伤的干预治疗提供理论依据。方法制各大鼠左后肢截肢创伤模型,将雄性Wistar大鼠80只随机分为10组:正常对照组,截肢后1h、2h、4h、6h、12h、24h、48h、72h组,FK506干预组,每组8只。测定肝肾组织CaNmRNA表达、血浆及肾组织肾损伤分子1(KIM-1)含量、肝肾功能指标(ALT、AST和Cr、BUN),观察肝、肾组织HE染色病理变化。结果与正常对照组相比,截肢创伤2h组肝脏、肾脏组织CaNmRNA即出现明显升高,于6h达峰值,分别为2.881±0.277(P〈0.01)、3.107±0.346(P〈0.01).72h恢复至正常水平:血浆及肾组织KIM-1含量于截肢创伤1h组即出现明显升高,至72h组持续升高.创伤6h组分别为1.650±0.194ug/L(P〈0.01)、0.921±0.071(P〈0.01);血浆ALT、AST和Cr含量均在创伤应激后出现明显升高(P〈0.05),血浆BUN含量无明显改变。FK506干预组与截肢6h组比较,肝肾组织CaNmRNA(1.003±0.210、0.856±0.158)、血浆及肾组织KIM-1含量(1.065±0.170ug/L、0.605±0.050)、血浆Cr含量均显著降低(P〈0.01,P〈0.05),血浆ALT、AST含量明显升高(P〈0.01),血浆BUN含量无明显变化。病理检查可见截肢6h组肝、肾组织充血及细胞肿胀明显,FK506干预后相对减轻。结论大鼠截肢创伤应激后可引起肝、肾组织结构和功能的损伤,FK506干预可以对这种损伤起到一定的防护作用。

Objective To investigate the liver and kidney injure of rats after traumatic amputation and the protective effect of FK506 on these tissues. Methods The rat models with amputation were prepared.
