高血压并发脑卒中的危险因素分析

目的：探讨高血压并发脑卒中的危险因素。方法：选择＂高血压病早期管理对心脑血管事件发生率的影响＂首发基金项目首钢地区的15年以上高血压患者1 628例,2009年4~7月对上述人群进行复查,将高血压伴脑卒中患者361例,作为Ⅰ组,将高血压不伴脑卒中患者1 267例,作为Ⅱ组。采用问卷调查、体格检查、实验室检查方法收集相关数据,采用Logistic回归对数据进行相关性分析。结果：Ⅰ组年龄、体重指数、吸烟率、血脂异常率及糖尿病患病率均显著高于Ⅱ组,差异有统计学意义（P〈0.05）,FBG、CRP、TC、TG、HDL-C、HDL-C等水平比较,两组差异无统计学意义（P〉0.05）。进行多因素Logistic回归分析显示,年龄和血脂异常与脑卒中发病密切相关,OR值分别为1.06和2.13。结论：对高血压患者进行早期管理,进行健康生活方式指导,可以提高患者的生活质量,降低脑卒中的发生率,使人群获益并为脑卒中的预防提供指导意义。

Analysis the risk factors of hypertensive patients combined with stroke

Objective： To analyze the risk factors of hypertensive patients combined with stroke.Methods： 1 628 cases of patients with hypertension for 15 years in shougang community from the ＂Effect on Cardiovascular and Cerebrovascular Events by early Hypertension Intervention＂ study were selected.Medical check was given to this cohort again from April to July 2009,and hypertensive patients combined with stroke were divided into group Ⅰ（n=361）,hypertensive patients without stroke were divided into group Ⅱ（n=1 267）.Questionnaire survey,physical examination and laboratory examination were used to select relevant data.Logistic regression was adopted to proceed data analysis.Results： Age,BIM,smoking rate,dyslipidemia rate and diabetes morbidity rate of group Ⅰwere higher than those of groupⅡ,the differences were statistically significant（P0.05）.Level of FBG,CRP,TC,TG,HDL-C,HDL-C were compared between groupⅠand groupⅡ,the differences were statistically significant（P0.05）.Logistic regression analysis showed that age and lipid disorders were closely related with stroke（OR=1.06,2.13 resectively）.Conclusion： Early management of patients with hypertension,healthy lifestyle guidance can improve the patients life quality and reduce the incidence of stroke,in order to provide guidance for stroke prevention.
