中文版护理谵妄筛查量表的信度和效度研究

目的考查中文版护理谵妄筛查量表在国内麻醉恢复室使用中的信度和效度。方法根据国际药物经济学与结果研究协会指南,对国际通用的护理谵妄筛查量表(Nu-DESC)进行编译,制定中文版并对228例手术后麻醉恢复室留观患者进行谵妄评估,测量其信效度。结果中文版Nu-DESC总分的组内相关系数值为0.91(P0.01)。与金标准DSM-Ⅳ对照,应用3分作为分界值时,灵敏度为0.80(95%CI:0.63-0.91),特异度为0.92(95%CI:0.87-0.95),两者的临床诊断一致性的Kappa值为0.66(P0.01)。结论中文版Nu-DESC具有满意的信度和效度,可作为麻醉恢复室谵妄筛查工具。

The validity and reliability of the Chinese version of Nursing Delirium Screening Scale

Objective To assess the reliability and validity of the Chinese version of Nursing Delirium Screening Scale(Nu-DESC).Methods The translation of Nu-DESC was done according to the ISPOR Task Force for Translation and Cultural Adaptation.In the validation process,the Nu-DESC Simplified Chinese Version was tested in 228 patients in post anesthesia care unit.DSM-Ⅳ served as gold standards.Results A forward translation of Nu-DESC was developed from 3 independently devised translations,a back-translation was then conducted by a registered state-approved translator.The back-translation of Nu-DESC was authorised by the author of the original.On the basis of the satisfactory results of cognitive debriefing,the translation process was finalised and a final of Nu-DESC Simplified Chinese Version(Nu-DESC-SCV)was made.The ICC between two examiners for the 5 items of Nu-DESC was from 0.66 to 0.85,the ICC for the all Nu-DESC was 0.91(P＜0.01).The sensitivity of Nu-DESC was 0.80(95%CI:0.63-0.91),and the specificity was 0.92(95%CI:0.87-0.95)when a cut of 3 was set by ROC analysis.The overall cross-validated Kappa was 0.66(P＜0.01).Conclusion Nu-DESC-SCV is a choice for delirium screening for postoperative and ICU patients in Chinese population.
