老年股骨颈骨折内固定术后颈短缩发生的影响因素分析

目的探讨老年股骨颈骨折内固定术后颈短缩的影响因素,以期为临床治疗提供参考。方法 2009年6月～2011年10月收治120例老年股骨颈骨折患者,均给予克氏针内固定治疗,并进行随访,根据患者的X线片表现,总结颈短缩的发生率和影响因素,并进行统计学分析。结果 120例患者均获随访,51例发生颈短缩,骨折愈合率为93.33%,髋关节Harris评分颈短缩组为（79±16）分,无颈短缩组为（85±24）分,两组比较差异有统计学意义（P〈0.05）。颈短缩组和无颈短缩组患者的年龄、性别、骨密度值与颈短缩的发生呈明显的正相关。结论老年患者股骨颈内固定术后颈短缩的发生率较高,患者的年龄、性别、骨密度值均为其影响因素,颈短缩对患者的骨折愈合无影响。

Influence of femoral neck shortening after fracture fixation in the aged patients

Objective To explore the old femoral neck fracture fixation postoperative neck shortening of the influencing factors,so as to provide reference for clinical treatment.Methods 120 elderly patients with fracture of the femoral neck in our hospital from June 2009 to October 2011,all patients were given kwire fixation treatment and follow-up,according to the patient′s X-ray performance,the incidence of neck shortening and influencing factors were sum up,and the results were statistical analyzed.Results 120 patients had been followed up,51 cases happened neck shortening,the fracture healing rate was 93.33%,Harris hip score was(79±16) points in neck shortening group,but in no neck shortening group was(85±24) points,difference had statistical significance between the two groups(P < 0.05).The patient′s age,gender,bone mineral density value and the occurrence of neck shortening were obviously positive correlation between the neck shortening group and no neck shortening group.Conclusion Elderly patients femoral neck shortening the internal jugular fixed a high incidence,the patient′s age,gender,bone mineral density value are the impact factors,neck shortening has no effect on the patients with fracture healing.
