采用HA130型树脂灌流器血液灌流对维持性血液透析患者微炎症的影响

摘    要:
背景:研究发现维持性血液透析的尿毒症患者体内存在“微炎症状态”。目的:观察采用HA130型树脂灌流器行血液灌流对维持性血液透析患者微炎症的影响。 方法:选取维持性血液透析治疗的尿毒症患者60例,随机分为2组,灌流组采用HA130型树脂灌流器串联血液透析器进行血液灌流加血液透析治疗;透析组行单纯血液透析治疗。采集治疗前后两组患者的血清,检测高敏C-反应蛋白、肿瘤坏死因子α和白细胞介素6水平。 结果与结论:灌流组血清高敏C-反应蛋白、白细胞介素6和肿瘤坏死因子α水平治疗后较治疗前显著降低(P < 0.05);透析组治疗后上述指标较治疗前略升高,差异无显著性意义。提示血液灌流可以降低维持性血液透析患者血清高敏C-反应蛋白、白细胞介素6和肿瘤坏死因子α水平,有利于减轻其微炎症反应。

Abstract:
BACKGROUND:Studies have found that the microinflammatory state exists in uremic patients undergoing maintenance hemodialysis (MHD).OBJECTIVE:To investigate the effects of hemoperfusion using HA130 resin hemoperfusion apparatus on microinflammatory state in MHD patients. METHODS:Sixty uremia patients undergoing maintenance hemodialysis were divided into two groups: hemoperfusion group (n=30) and MHD group (n=30). Hemoperfusion group underwent the treatment of hemoperfusion using HA130 resin hemoperfusion apparatus combined with hemodialysis. MHD group underwent the treatment of hemodialysis. The serum levels of high-sensitivity C-reactive protein (hs-CRP), interleukin-6 (IL–6) and tumor necrosis factor alpha (TNF-α) of all patients in the two group were detected before and after the treatment. RESULTS AND CONCLUSION:In the hemoperfusion group, the serum levels of hs-CRP, IL-6 and TNF-α were lower after treatment than those before treatment, and the difference was significant (P < 0.05); In the MHD group, the serum levels of hs-CRP, IL-6 and TNF-α were higher after treatment than those before treatment, and the difference was not significant (P > 0.05). The results indicate that hemoperfusion using HA130 resin hemoperfusion apparatus can reduce the serum levels of hs-CRP, IL-6 and TNF-α and control the microinflammatory state in MHD patients.
