彩色多普勒与糖尿病下肢动脉血管病变辨证分型的相关性研究

目的：探讨彩色多普勒超声与糖尿病下肢动脉血管病变中医辨证分型的相关性。方法：对446例糖尿病下肢动脉血管患者按中医辨证分为4组：阴血两虚、皮肤失养证（A组）,气虚血瘀、肌肤脉络痹阻证（B组）,阴虚血瘀、肌肤毒聚阴疡证（C组）,湿热瘀阻、肌肤筋骨毒腐证（D组）。各组分别进行彩色多普勒下肢动脉血流显像检查,观测各组下肢动脉二维及彩色多普勒频谱显像情况。结果：A、B组下肢动脉病变多以内膜增厚、毛糙、小斑块形成为主,C、D组病变多以闭塞、血管弹性、血流频谱、血流速度等血流动力学改变为主。结论：彩色多普勒超声与糖尿病下肢动脉血管病变中医辨证分型具有较高的相关性。

Research on the Relationship Between Color Doppler Ultrasound and Differentiation of the Syndromes of Diabetic Lower-Extremity Arterial Disease

Objective ：To explore the relationship between color Doppler ultrasound （CDU） and differentiation of the syndromes of diabetic lower-extremity arterial disease （DLEAD）. Methods：446 cases of DLEAD were divided into 4 groups according to TCM syndrome differentiation as follows：syndrome of deficiency of both Yin and blood, skin dystrophy（group A） ;syndrome of Qi deficiency and blood stasis, skin vessels and collaterals obstruction （group B） ;syndrome of Yin deficiency and blood stasis, toxin accumulation causing skin infections （group C） ;syndrome of accumulation of damp-heat and blood stasis,skin bone and muscules toxin and corrosion（group D）. The two-dimensional image and color Doppler image of lower limb artery of each group were observed. Results：The pathological changes in group A and B were mainly endomembrane thickness,roughness and small plaques;pathological changes in group C and D were mainly hemodynamics changes of blockage, flexibility of blood vessels, blood flow frequency spectrum, blood flow velocity, etc. Conclusion ： Syndrome differentiation of DLEAD was correlated with CDU.
