摘    要:目的 探讨2型糖尿病住院患者医院感染的临床特点,分析抗菌药物使用情况,为更有效预防和控制医院感染提供科学依据.方法 回顾性分析医院内分泌科2010年2型糖尿病住院患者840例相关临床资料.结果 840例患者中64例发生医院感染,感染率为7.6%;感染部位以呼吸系统为主28例,占43.8%,其次为泌尿道21例,占32.8%;医院感染患者的病原学送检率为75.0%,病原菌以革兰阴性菌为主;年龄、住院天数、病程,糖化血红蛋白等是医院感染的高危因素;医院感染的抗菌药物使用存在不合理现象.结论 糖尿病患者医院感染率较高,需预防与控制高危因素降低感染率,并针对医院感染合理使用抗菌药物.

Abstract:OBJECTIVE To investigate the situation of nosocomial infections in type 2 diabetes patients and analyze the status of the use of antibiotics so as to provide scientific basis for effective prevention and control of nosocomial infections.METHODS The clinical data of 840 hospitalized patients with type 2 diabetes in department of endocrinology in 2010 were retrospectively analyzed.RESULTS The infection rate was 7.6%(64 infection cases in 840 cases).The main infection sites were respiratory tract(28 cases,43.8%),and urinary tract(21 cases,32.8%);the submission rate of the etiology was 75.0%,gram-negative bacteria were the main pathogens;the high risk factors for nosocomial infections included the age,hospital days,diabetes duration and glycosylated hemoglobin(HbA1c);the use of antibiotics was irrational.CONCLUSION Nosocomial infection is frequent in diabetes patients.It’s necessary to prevent and control the risk factors to lower the infection rate and use antibiotics scientifically.
