外科围手术期抗菌药物应用研究

目的 调查普外科围手术期抗菌药物的应用情况,分析、评价其用药合理性.方法 随机抽取2010年6-12月出院的外科手术患者病历268份,对抗菌药物的种类、用法用量、手术时间、用药频度、药物利用及药物联用等进行统计与分析,计算用药频度,评价其合理性.结果 全部手术患者均应用抗菌药物,其中单用159例占59.33％,二联102例占38.06％,三联7例占2.61％,无＞3种抗菌药物联用现象；前10位抗菌药物中主要品种是头孢菌素类；普外科围手术期存在用药疗程偏长、选药起点偏高等不合理用药情况.结论 普外科围手术期用药基本能达到预防手术部位感染目的,但还需要加深临床医师合理应用抗菌药的意识,加强外科围手术期合理使用抗菌药的教育与管理.

Application of antibiotics during perioperative period of surgery

OBJECTIVE To investigate the situation of the use of antimicrobials during perioperative period of general surgery,and analyze and evaluate the rationality of the use of antibiotics.METHODS A total of 268 medical records of the patients who discharged from Jun to Dec 2010 were randomly selected;the types of antibiotics,dosing,operation duration,DDDs,and combination use of antibiotics were analyzed for statistics,the DDDs was calculated,and the rationality was evaluated.RESULTS All the patients undergoing surgery have used antibiotics,among which 159(59.33%) patients used single drug,102(38.06%) patients used two-drug,and 7(2.61%) patients used three-drug;no patients used more than three-drug;the top 10 types of antibiotics used were the main cephalosporins;such irrational use of antibiotics as overlong course of antibiotics treatment and high starting points for drug selection existed during perioperative period of general surgery.CONCLUSION The use of antibiotics during perioperative period can achieve the purpose of prevention of surgical sites infections,but it remained necessary to further strengthen the awareness of reasonable use of antibiotics and intensify the management of reasonable use of antibiotics during perioperative period of general surgery.
