膀胱冲洗频率与速度对脊髓损伤患者尿路感染相关性调查分析

目的 探讨长期留置导尿管患者膀胱冲洗频率与速度的最佳选择及与尿路感染的关系,为预防尿路感染提供依据.方法 取2010年3月-2010年12月医院骨科病房脊髓损伤需长期留置导尿住院患者共60例,随机分为对照组和试验组,每组30例；膀胱冲洗频率:试验组2次/每周,对照组1次/2 d；试验组再分为A组,膀胱冲洗速度以80～100滴/min速度冲洗,B组,以120～150滴/min速度冲洗,分别于留置导尿后第14、21、28天进行尿细菌培养检查再进行比较分析.结果 试验组尿路感染率高于对照组(P＜0.05),试验组膀胱冲洗速度A、B组尿路感染率差异无统计学意义.结论 对于脊髓损伤需长期留置导尿患者,其膀胱冲洗频率越频繁,尿路感染率越高,而增加膀胱冲洗速度对尿路感染率影响不大.

Correlation between bladder irrigation frequency and speed and urinary tract infections in patients with spinal injury

OBJECTIVE To investigate the choice of bladder irrigation frequency and speed in long-term indwelling catheter patients with spinal injury and research the relationship between of bladder irrigation and urinary tract infections so as to provide basis for prevention of urinary tract infections.METHODS A total of 60 patients with spinal injury admitted to the hospital from Mar,2010 to Dec,2010 were studied.The patients were randomly divided into the tested group and the control group with 30 patients in each group.Bladder irrigation frequency:the tested group twice per week,and control group once every other day.Then the tested group was divided into A1group with irrigation speed 80-100 drops/min and A2 group with irrigation speed 120-150 drops/min.The urine culture was performed respectively on the14th,21st,and 28th day of the indwelling catheterization.RESULTS The incidence rate of the urinary tract infections was higher in the tested group than in the control group(P<0.05).The difference in the incidence rate of urinary tract infections between the irrigation speed A group and the irrigation speed B group in the tested groups was not statistically significant.CONCLUSION For the patients with spinal injury undergoing long-term indwelling catheterization,the more frequent the bladder irrigation is,the higher the incidence rate of the urinary tract infections,and the increase in the bladder irrigation speed had little effect on the incidence of urinary tract infections.
