钛网在颌骨粉碎性骨折复位中的应用

摘    要:
目的:评估钛网在颌骨粉碎性骨折复位中的应用。方法:选择2002年12月至2004年6月收治的颌骨粉碎性骨折患者12例,常规手术径路。选用内固定复位材料包括钛网(0.6mm),微型/小型钛板(0.9~1.5mm)和单、双层皮质骨钛螺钉。结果:本组手术均为一期愈合.外形恢复满意,其中10例完全恢复咬合关系:术后X线检查,钛网固定骨折碎片对位良好,未见螺钉松脱、愈合不良或骨不粘连。结论:1)钛网应用于颌骨粉碎性骨折较钛板具有更好的三维稳定性,并且易于操作;2)钛网对骨折碎片的覆盖固定作用,利于维持颌骨外形完整;3)对于颌骨缺损者,钛网作为整形重建的支架,具有良好的塑形作用;4)在颌骨功能荷重区,钛板较钛网更具固位支持力。

Abstract:
Objective: To evaluate the effect and reliability of internal fixation of comminute fracture of jaws using Ti mesh.Methods: Between Dec.2002 and Jun 2004 12 comminuted fracture of the jaws were fixed using Ti mesh(0.6mm),mini/micro-plates(0.9~1.5mm) and Ti Screw systems.Results: Primary wound-healing was achieved and union occurred without mesh-related complication and malocclusion in this group by postoperative X-ray shown.Conclusions:1) Three-dimensional stabilization of fragment achieved with Titanium mesh seemed better than mini-plates in comminuted bone fractures.2) Bone grafts and fragments could be inserted and fixed by using Ti mesh which meant functionally and aesthetically satisfactory restoration of bony continuity.3) Ti mesh was being used to reconstruct the rigid framework of the jaws and cover bone defects.4) Mini-plates could be used to resist the move of fragments in loading area of bony fractures.
