摘    要:目的:总结血管内支架结合电解可脱卸弹簧圈治疗颅内椎动脉宽颈动脉瘤的初步经验。方法:通过股动脉途径,采用将血管内支架跨动脉瘤颈放置,通过其网孔将微导管送入动脉瘤腔后填塞GDC的技术治疗6例椎动脉颅内段宽颈动脉瘤。结果:1例支架置入术失败,其余5例支架均准确到位。4动脉瘤达到致密填塞,1例大部填塞,载瘤动脉通畅。1例采用弹簧圈同时闭塞动脉瘤及载瘤动脉,无手术并发症,患均恢复良好。结论:血管内支架联合微弹簧圈是治疗颅内宽颈动脉瘤的有效方法,但仍需要远期随访。

Abstract:Objective: To summarize the early outcome of combined endovascular treatment of intracranial vertebralwide-necked aneurysms with stent and Guglielmi detachable coils (GDC). Methods: The coronary stents were implanted acrossthe necks of 6 intracranial vertebral wide-necked aneurysms via femoral route. Microcatheters were introduced into the a-neurysmal sac through the stent mesh,and GDCs were used to embolize the aneurysms. Results: The stents were deployed inall cases except failure in one case because of tortuous parent artery. Total occlusion was obtained in 4 cases,while more than90% in 1 case ,all the parent arteries maintained patency. One aneurysm ,together with parent artery was occluded. All the pa-tients recovered well without any procedural complications. Conclusion: Endovascular therapy combining stent implantationand microcoil placement may be a valid alternative for the treatment of intracranial wide-necked aneurysms. Experience intreating greater numbers of patients and long-term follow-up are required to further validate this technique.
