碘化油在离体子宫肌瘤声学显影特性的实验研究

摘    要:
目的 探讨碘化油在离体子宫肌瘤组织内的显影效果及弥散规律,为碘化油联合高强度聚焦超声(HIFU)治疗子宫肌瘤提供理论依据和实验数据.方法 将40枚离体子宫肌瘤随机均分为实验组和对照组,实验组瘤体中心注入碘化油1 ml,记录注入后即刻,10 、20 、30 、40 、70 、110 min瘤体中碘化油形成的团状强回声的超声灰度值和灰度面积;对照组以生理盐水代替碘化油,除不测定B型超声灰度面积外,余同实验组.结果 实验组碘化油注入即刻,B型超声灰度值最大(75.1±7.7)dB,然后逐渐下降、消失.对照组注入生理盐水后灰度未见明显增加,也未表现出随时间变化的规律性.实验组B型超声灰度面积在碘化油注入后呈类似同心圆增大,至30 min达最大值,后逐渐变小、消失.结论 碘化油在离体子宫肌瘤组织中具有明显的声学显影作用和缓慢的弥散性,该特性可用于碘化油联合HIFU治疗子宫肌瘤的实际临床应用中.

Abstract:
Objective To explore the acoustic property and diffusion manner of iodized oil (IO) in hysteromyoma in vitro, and to further provide theory basis and experimental data for treatment of hysteromyoma by combining high intensity focused ultrasound (HIFU) and IO in clinic. Methods Forty ex vivo hysteromyoma were equally divided into experimental group and control group. 1 ml of IO was injected into the center of hysteromyoma in experimental group, the grayseale value(GV) and grayseale area(GA) formed by IO were recorded by B-gray-processing software attached on HIFU system immediately( 0 min), 10 min,20 min, 30 min,40 min,70 rain and 110 min after IO was injected. All processes were as the same as those in experimental group except that IO was replaced by physiologic saline and GA was not measured in control group. Results The maximum GV( 75.1 ±7. 7 )dB in experimental group was observed immediately( 0 min) after IO was injected into hysteromyoma, meanwhile, GA was nearly circularly enlarged and maximum GA was recorded 30 min after IO was injected, then G~ and GA decreased gradually and disappeared finally. Conclusion IO posseses evident characteristics of acoustic visualization and slow diffusion manner in hysteromyoma in vitro, and these characteristics of IO could be further used in HIFU ablation of hysteromyoma by combined with IO in clinic.
