实时三维超声成像对胎儿先天性心脏病的诊断价值

摘    要:目的: 探讨实时三维超声成像对胎儿先天性心脏病的诊断价值。方法: 应用实时三维超声心动图和彩色多谱勒血流显像仪,对3 500例孕22~28周在常规二维超声检查中85例拟为胎儿先天性心脏病的孕妇又进行三维彩色多谱勒超声成像,在观察胎儿四腔心的基础上,增加心室流出道、三血管平面和短轴切面,进行必要的局部放大以观察心脏各切面结构和彩色血流特征。结果: 检查出的12例胎儿先天性心脏病中,其中室间隔缺损4例,缺损大小约4 mm,法洛四联症2例,单心室3例,左室发育不良1例,心内膜垫缺损1例,左室横纹肌瘤1例。结论: 实时三维超声心动图可对胎儿先天性心脏病进行多切面观察,图像丰富,形态自然、完整,具体部位结构显示更清晰,是对二维超声成像的有益补充,可提供更丰富的诊断信息。

Abstract:Objective:To investigate the diagnostic value of fetus congenital heart diseases with real time 3-dimensional echocardiography(3-DE).Methods:Three thousand and five hundred patients were referred for fetal echocardiography including four-chamber,outflow tracts,three vessels and short axis views.Characteristics of cardiac structures and color blood flow were observed with real time 3-DE Color Doppler Sonography.The local three-dimensional amplification and segmental analysis were used if necessary.Results:In echocardiographic findings,2 fetuses with prenatal diagnosis of congenital heart diseases were found.Among 12 fetuses,VSD was found in 4 with the mean defect size of 4 mm, teralogy of Fallot in 2,simple ventricle in one, cardiac rhabdomyoma of left ventricle in one, endocardial cushion defect in 3 and hypoplasia of the left ventricle in one fetus.Real time 3-DE could provide abundant information with the similar shapes and structure.Conclusions:Real time 3-DE may provide more diagnostic information than 2-DE and can serve as a beneficial supplement to 2-DE in multiple views for congenital heart diseases in fetus.
