剖宫产术中行子宫肌瘤剔除术160例临床分析

目的：探讨剖宫产同时行子宫肌瘤剔除术的可行性。方法：回顾性分析160例剖宫产术中同时行子宫肌瘤剔除术的产妇,与同期100例行单纯剖宫产术的产妇作为对照,比较两组的手术时间、出血量、住院天数。按肌瘤大小分为〉8cm肌瘤组与≤8cm肌瘤组,对其术中出血量、手术时间及术后住院天数进行比较。结果：研究组手术操作时间较对照组延长,差异有统计学意义（P〈0.05）;但两组的术中出血量和术后住院天数比较,差异均无统计学意义（P〉0.05）。〉8cm肌瘤组与≤8cm肌瘤组相比较,手术时间明显延长,术中出血量显著增多（P〈0.05）。结论：根据患者的具体情况,选择性地行剖宫产同时子宫肌瘤剥除术是安全可行的,对部位特殊的子宫肌瘤及大型子宫肌瘤的处理须谨慎。

Clinical analysis of 160 patients treated by myomectomy during cesarean section

Objective：To evaluate the efficacy of myomectomy during cesarean section.Methods： Control group（100 patients）underwent cesarean section,study group（160 pregnant women） underwent myomectomy during cesarean section.Operative time,intraoperative blood loss and postoperative inpatient period were analyzed retrospectively.According to the tumor size,patients were devided into 2 groups： leiomyoma bigger than 8cm group and the ≤8cm group.Results： The operative time of study group was significantly longer than that of control group（P0.05）.There was no significant difference between two groups in intraoperative blood loss and postoperative inpatient period（P0.05）.The group in which leiomyoma are bigger than 8cm needs significantly longer operative time and lost more blood than the other in which leiomyomas were ≤8cm.Conclusion： According to the specific conditions of the patient,selective myomectomy during cesarean section is safe,but when leiomyoma is bigger or locating at a special position,myomectomy during cesarean section should be cautious.
