原发不孕行体外受精-胚胎移植术授精方式的探讨

目的　分析原发不孕行体外受精 胚胎移植术 (IVF ET)患者的受精及妊娠率 ,为今后同类患者选择手术方式提供参考。方法　将 14 0例原发不孕患者分为输卵管性不孕组 10 1例 ,原因不明不孕组 39例 ,行IVF ET ,比较两组间受精率、受精失败率及临床妊娠率。结果　原因不明不孕组受精率明显低于输卵管性不孕组 (P <0 0 1) ,而受精失败率明显高于后者 (P <0 0 1) ,二者临床妊娠率差异无显著性意义。结论　对原发不孕输卵管性不孕患者IVF ET可作为首选。受精率低是影响原因不明不孕的重要因素 ,施行常规体外受精和胞质内单精子注射 (ICSI)各一半的授精方式 ,是此类患者的首选。

Methods of Insemination for IVF-ET Patients with Primary Infertility

Objective To analyze the fertilization rate and pregnancy rate of IVF-ET patients with primary infertility and infer the feasible insemination method for these patients. Methods 140 patients with primary infertility were divided into two groups: tubal infertility group (n=101) and unexplained infertility group (n=39). The fertilization rate, fertilization failure rate and clinical pregnancy rate were compared between the two groups. Results The fertilization rate in unexplained infertility group was significantly lower than that in tubal infertility group (P<0.01), while the fertilization failure rate in unexplained infertility group was obviously higher than that in tubal infertility group (P<0.01). There was no significantly statistical difference in clinical pregnancy rate between the two groups. Conclusion IVF was the first choice for the patients with tubal primary infertility. Low fertilization rate was the important factor of patients with unexplained primary infertility and IVF-ICSI split may be a better choice for these cases.
