清开灵注射液致过敏性休克58例文献分析

目的探讨清开灵注射液引起过敏性休克的原因与规律。方法检索1994年1月至2007年6月国内医学期刊报道的清开灵注射液所致过敏性休克病例文献，并进行统计分析。结果收集到58例清开灵注射液所致过敏性休克患者的资料，其中男26例（44．83％）。女32例（55．17％），18～44岁的患者数量最多（53．45％）；原发疾病以上呼吸道感染比例最高（79．31％）；用药剂量多在20～40mL范围内；有输入时间记载的过敏性休克均发生在给药后30min内，其中72．92％发生在10min以内；58例中有3例死亡，其余患者经积极抢救治疗均好转、痊愈。结论清开灵注射液可引起较严重甚至危及生命的过敏性休克反应，其发生机制可能与患者体质、药物本身成分和辨证不准等因素有关，临床应用中应遵循中医理论辨证施治，减少不必要的配伍联用，并加强用药监护，以避免不良反应的发生。

58 Cases of Literature Review of Anaphylactic Shock Induced by Qingkailing Injection

Objective To investigate the causes and rules of anaphylactic shock caused by Qingkailing Injection. Methods Qingkailing Injection induced anaphylactic shock cases reported in the domestic pharmaceutical journals in Jan. 1994-Jun. 2007 were retrieved and analyzed statistically. Results The male cases were 26 （44. 83% ） and the female cases were 32 （55.17%） in the 58 cases with gender recordation. There were a large number （53.45%） of young sufferers from age 18 to 44 in the cases reported. In primary disease,the upper respiratory tract infection rate was up to the highest 79. 31%. The dosage of Qingkailing Injection was in the range of 20-40mL, the occurrence of anaphylactic shock was commonly within 30 min after injected,and 72.92% was within 10 min. The anaphylactic shock patients were cured after emergency treatment except 3 were dead. Conclusion Qingkailing Injection can induce dangerous even fatal anaphylactic shock, its mechanism may be related to the constitution of the patient, ingredient of the drug itself and diagnosis warp. The treatment based on syndrome differentiation combined with traditional Chinese medicine theory should be adhered to,unnecessary compatibility should be decreased and the monitor of the drug use should be strengthened in the clinical application for avoiding the occurrence of adverse effects.
