CT介导下经皮盐酸注射疗法原位灭活原发性肝癌的临床研究

目的:探讨CT介导经皮盐酸注射疗法对肝癌的原位灭活作用与临床安全性。方法:应用CT介导下经皮穿刺对原发性肝癌35例48个瘤灶内注射含6mol/L盐酸的注射液,观察原位灭活率、影像学组织学变化、远期生存率和不良反应。结果:CT扫描显示肿瘤呈低密度改变,肿瘤血管消失,PET检查肿瘤失去代谢,穿刺活检全部为坏死组织,肿瘤坏死彻底,AFP降低;全组35例患者均随访,1年生存率为100%,2年生存率为91%,3年生存率为64%,其中3例生存超过4年。死亡原因主要是其他部位的复发转移。主要的不良反应包括轻度疼痛、低热。结论:CT介导下经皮盐酸注射疗法治疗肝癌可原位灭活癌组织,是非手术局部治疗肝癌的新方法,值得进一步研究。

Clinical study of CT mediated percutaneous hydrochloric acid injection in situ destructive therapy for primary liver cancer

Objective:To evaluate in situ destructive effects and safety of CT mediated percutaneous hydrochloric acid injection therapy for liver cancer.Methods:Guidance and monitored by CT slice simulation puncture technique,35 patients with 48 malignant lesions of primary liver cancer were treated intratumor injection of a protein destructive agent contained 6mol/L hydrochloric acid.The in situ destructive rate,imaging and histopathology evaluation,survival rate and side effects were observed.Results:A low density and no enhancement of tumor by CT scan were shown.A well necrosis and destructive rate of cancer was obtained by PET,biopsy and AFP measurement.The overall 1-,2-,and 3-year survival rates were 100%,91% and 64%,respectively.Three cases of all patients had survived over 4 years.The reason of death was recurrence and metastasis.No changes of hart,liver and kidney function were fund.Mean side effects were slight pain,lower fever and so on.Conclusion:CT mediated percutaneous hydrochloric acid injection therapy(PHAIT)shown well in situ destructive effects and was a new method for percutaneous treatment of the liver cancer.
