蛛网膜下腔舒芬太尼硬膜外镇痛联合小剂量异丙酚预防分娩镇痛瘙痒的效果及安全性

目的 探讨蛛网膜下腔舒芬太尼硬膜外患者自控镇痛（PCEA）复合应用小剂量异丙酚预防分娩镇痛所致瘙痒的临床效果及安全性.方法 取84例自愿实施分娩镇痛的单胎足月初产妇,将其分为对照组与观察组各42例,在腰硬联合阻滞麻醉前给予对照组20mg生理盐水、观察组20mg丙泊酚静脉注射.结果 两组镇痛效果及镇痛起效时间无显著差异（P＞0.05）;组间产程、剖宫产率及胎儿Apgar评分无显著差异（P＞0.05）;观察组瘙痒发生率及严重程度低于对照组（P＜0.05）.结论 丙泊酚预防蛛网膜下腔舒芬太尼复合PCEA进行分娩镇痛引起瘙痒,同时不影响产程及母婴健康,效果确切,值得推广应用.

Efficacy and safety of subarachnoid sufentanil PCEA and small dose of propofol for prevention of labor analgesia induced pruritus

Objective To explore the efficacy and safety of subarachnoid sufentanil PCEA and small dose of propofol for prevention of labor analgesia induced pruritus. Methods A total of 84 cases of voluntary implementation of labor analgesia single fetus primiparae were randomly divided into two groups： the control group and observation group with 42 cases in each. Before the combined spinal-epidural anesthesia, the control group were given maternal physiological 20rag saline, the observation group were given 20rag propofol intravenously. Results The two groups of patients in the analgesic effect and the timing of onset of analgesia was not significantly different （P 〉0.05）; groups of labor, cesarean section rate and fetal Apgar scores also no significant difference （P 〉0.05）; the observation group incidence of pruritus and severity were lower than the control group （P〈0.05）. Conclusion Propofol in patients with subarachnoid sufentanil analgesia during PCEA help prevent itching occur, without affecting the production process as well as maternal and child health, the exact effect, should be widely applied.
