创伤失血性休克的急救与时间窗分析

摘    要:目的探讨急诊创伤失血性休克患者中救治时间窗与患者预后的关系。方法选取本院2011年1月~2013年1月收治的90例急诊创伤失血性休克患者,按照送院时间分为对照组和治疗组,其中对照组入院时间为4~10 h,治疗组为1 h左右。两组患者在送入我科前均接受了普通的急救处理,比较两组间接受针对性手术处理后的救治结局和住院情况。结果两组在运输途中死亡人数间的差异无统计学意义(P>0.05),而对照组中抢救无效死亡人数、手术后死亡人数、手术后康复人数对比治疗组分别高19.13%、13.87%、35.83%。治疗组患者的住院情况明显优于对照组,差异具有统计学意义(P<0.05)。结论严重的多发伤常会造成创伤失血性休克,使机体有效循环血量的急剧减少造成机体组织器官功能障碍,所以在"黄金1h"内进行及时有效的手术治疗对于患者的预后是非常关键。

Abstract:Objective To study the relationship between the treatment time window and prognosis of patients with traumatic hemorrhagic shock. Methods Divided 90 patients into control and treatment groups according to the time into our hospital. The time of control group was 4-10 hours, and that of treatment group was about 1 hour. Both groups accepted general first aid treatment before arrived at our department. Results In the control group and the treatment group differences between the death toll has no statistical significance in transit (P 〉0.05), while in the control group died after operation, the number of deaths, the number of operation after rehabilitation treatment group compared the 19.13%, 13.87%, 35.83% respectively. Comparison of two groups of hospitalization, treatment differences in clinical groups were higher than those in control group was statistically significant (P 〈0. 05). Conclusion Effective surgical treatment in the "golden one hour" is very critical for the prognosis of patients with traumatic hemorrhagic shock.
