早期护理干预在部分足拇趾再造缺损指尖术后血管危象中的作用

目的探讨早期护理干预在部分足拇趾再造缺损指尖术后血管危象中的作用。方法回顾性分析我院病房部分足拇趾再造缺损指尖术后患者82例，分为对照组42例、干预组40例。对照组给予手足外科常规护理，干预组针对足拇趾再造缺损术后实施早期护理干预。结果对照组动脉危象2例，静脉危象3例，动静脉危象3例，干预组动脉危象0例，静脉危象1例，动静脉危象1例，干预组较对照组血管危象明显下降（X2=-10．66，P〈0．01）；对照组术后早期血管危象5例，术后晚期血管危象3例，干预组术后早期血管危象2例，术后晚期血管危象0例，干预组术后早期和晚期血管危象均明显减少（x^2=4．10，P〈0．05）；对照组住院时间（12．53＋2．14）d，干预组住院时间（8．17±2．09）d，两组比较差异有显著性（t=9．30，P〈0．01），对照组住院费用（18983．53±1391．14）元，干预组住院费用（15362．02±1281．37）元，两组比较差异有显著性（t=12．27，P〈0．01）；对照组病人满意24例，不满意18例，干预组病人满意度31例，不满意9例，丽组比较差异有显著性（X2=4．10，P〈0．05）。结论早期护理干预能有效降低足拇趾再造缺损指尖术后血管危象的发生率，缩短住院时间，减少医疗费用，提高病人满意度。

Effect of early nursing interventions on vascular crisis after reconstruction of partial of the great toe deformity

Objective To study effect of early nursing interventions on vascular crisis alter reconstruction ot partial ot the great toe deformity. Methods Eighty-two children after reconstruction of partial of the great toe deformity were an- alyzed retrospectively, divided into intervention group （n = 40） and control group （n = 42）. The control group adopted routine postoperative nursing of hand and foot surgery, and the intervention group adopted early nursing interventions besides routine postoperative nursing. Results In control group, two patients happened arterial crisis, three patients happened venous crisis, three patients happened arterial-venous crisis, and intervention group, no patient happened arterial crisis, one patient happened venous crisis, one patient happened arterial-venous crisis, the occurrence of vascular compromise significantly decreased in the intervention group than in the control group （X2=10.66,P 〈 0.01）. In control group, five patients happened vascular crisis in early stage after operation, and three patients happened vascu- lar crisis in advanced stage after operation, while in intervention group, two patients happened vascular crisis in early stage after operation, and no patient happened vascular crisis in advanced stage after operation, the occurrence of vascular crisis significantly decreased in early and advanced stage after operation in the intervention group than in the control group （Xz=-4.10,P 〈 0.05）. The hospitalized time in control group was （12.53±2.14）days, while the hospitalized time in intervention group was （8.17±2.09）days, there was significantly difference between them （t = 9.30,P 〈 0.01）. the hospitalization cost in control group was （18 983.53±1391.14）yuan, and which in intervention group was （15362.02± 1281.37）yuan, there was significantly difference between them （t = 12.27 ,P 〈 0.01）.In control group 9 cases were re- garded as satisfied and 18 cases as unsatisfied, while in intervention group 31 cases were regarded as satisfied and 9 cases as unsatisfied, there was significantly difference between them （~2=4.10,P 〈 0.05）. Conclusion The risk factors of vascular crisis effectively reduced,in-hospital duration decreased, the satisfaction of patient enhanced and medical ex- penses reduced by early nursing interventions after the reconstruction of partial of the great toe deformity.
