湖北地区育龄期男性不同类型慢性前列腺炎患者精液质量分析

目的:了解不同类型的慢性前列腺炎精液质量的变化,并评价不同类型前列腺炎对不育症的影响程度。方法:对全部入选的848例慢性前列腺炎患者按照WHO1999年标准进行诊断并按NIH前列腺炎分类方法分为4种亚型,即Ⅱ型、Ⅲa型、Ⅲb型、Ⅳ型,同时采用计算机辅助精子分析(CASA)技术进行精液分析。结果:各组间精液质量参数分析显示:Ⅱ型及Ⅲa型的活精子比例、一级向前运动精子占活精子比例均分别低于Ⅲb型(P<0.01)、Ⅳ型(P<0.01),Ⅱ型及Ⅲa型畸形精子比例则高于Ⅲb型(P<0.01)、Ⅳ型(P<0.01);而Ⅱ型与Ⅲa型、Ⅲb型与Ⅳ型组间则无显著差异。精液中白细胞数Ⅱ型与Ⅲa型(P<0.01)、Ⅲb型(P<0.01)、Ⅳ型(P<0.01)的差异均具有统计学差异,Ⅲa型也与Ⅲb型(P<0.01)、Ⅳ型(P<0.01)有统计学差异,但Ⅲb型与Ⅳ型组间无统计学意义。结论:不同类型慢性前列腺炎对精液质量改变的影响不同,其影响程度的比较为:Ⅱ型、Ⅲa型大于Ⅳ型、Ⅲb型。

Semen Quality Analysis About Different Types of Chronic Prostatitis in Men of Childbearing Age in Hubei Province

Objective: To observe the semen quality and its influence on infertility in different types of chronic prostatitis.Methods: A total of 848 patients were diagnosed as chronic prostatitis by the standard of World Health Organization(WHO) in 1999.And according to the National Institutes of Health(NIH) methods,they were divided into four sub-types including typeⅡ,type Ⅲa,type Ⅲb and type Ⅳ.The semen from these patients was analyzed by using computer assisted sperm analysis(CASA).Results: The proportions of competent sperm and the proportions of forward motility of sperm in typeⅡand type Ⅲa were respectively significantly lower than in type Ⅲb and type Ⅳ(P<0.01),and the proportions of malformed sperm in typeⅡand type Ⅲa were respectively significantly higher than in type Ⅲb(P<0.01) and type Ⅳ(P<0.01).But there was no significant difference between type Ⅱ and type Ⅲa,or between type Ⅲb and type Ⅳ in the above indices.The WBC count in type Ⅱ and type Ⅲa was significantly more than that in typeⅢb and type Ⅳ(all P<0.01).Differences were found between type Ⅱ and type Ⅲa,but were not found between typeⅢb and type Ⅳ.Conclusion: The influence of typeⅡ and type Ⅲa of chronic prostatitis on semen quality is much more than that of type Ⅳ and type Ⅲb.
