山莨菪碱对肠缺血再灌注大鼠肠黏膜血流量的影响

摘    要:目的:探讨山莨菪碱能否改善肠缺血-再灌注损伤时的肠黏膜血流。方法:SD大鼠分为3组,先制作空肠袋,然后将激光多普勒探头和肠黏膜张力计旋转在空肠袋两端,用动脉夹阻断肠系膜上动脉血流60分钟后,再恢复灌流90分钟。于恢复灌流即刻分别向空肠袋内注射山莨菪碱、多巴酚丁胺和生理盐水,测定肠黏膜血流量和局部二氧化碳分压(PrCO2)。结果:恢复灌流过程中,与生理盐水组比较,山莨菪碱和多巴酚丁胺组肠黏膜血流量显著增加,肠黏膜PrCO2降低。山莨菪碱组与多巴酚丁胺组比较,再灌注30和60分钟时肠黏膜血流量和PrCO2无显著差别;但再灌注90分钟,山莨菪碱组肠黏膜血流量显著高于多巴酚丁胺组,PrCO2低于多巴酚丁胺组.结论:肠缺血后恢复灌流时,肠内给予山莨菪碱能增加肠黏膜血流量,降低肠黏膜PrCO2,作用较多巴酚丁胺持久。

Abstract:Objective:To determine if anisodamine treatment could improve alterations in mucosal perfusion during gut ischemiareperfusion (I/R).Methods:Sprague Dawley rats had either a laser Doppler probe or a tonometer inserted into a jejunal sac filled saline followed by 60 minutes of superior mesenteric artery occlusion (SMAO) and 90 minutes of reperfusion.At the end of 60 minutes of SMAO either 0 2 mg/kg of anisodamine or dobutamine was injected into the jejunal.Laser Doppler mucosal blood flow and regional pressure of cabon Oxide (PrCO 2) measurements were obtained.Results:Mucosal blood flow was significantly increased during reperfusion when intraluminal anisodamine or dobutamine was present as compared to intraluminal saline only.Blood flow changes were reflected by lower jejunal PrCO 2 measurements with intraluminal anisodamine or dobutamine.Most interesting finding was that there were significantly lower jejunal PrCO 2 and high mucosal blood flow in anisodamine group than those in dobutamine at 90 minutes of reperfusion,which suggested that anisodamine had longer lasting effect on mucosal perfusion than dobutamine.Conclusions:Intraluminal anisodamine can augment mucosal blood flow during gut I/R and may provide the protective effect on gut from ischemiareperfusion injury.
