直肠后壁庤上黏膜保留切除吻合术与Milligan-Morgan术治疗混合痔的临床疗效比较

【目的】探讨吻合器直肠后壁痔上黏膜保留切除吻合术(SRMEB)治疗混合痔的临床疗效。【方法】回顾性分析2017年1?12月本院收治的90例混合痔患者临床资料,根据手术方法的不同将其分为观察组(采用SRMEB术)和对照组(采用外剥内扎术),每组45例。比较两组手术情况、临床疗效及术后恢复情况。【结果】观察组痊愈42例、显效3例,对照组痊愈41例、显效5例,两组临床有效率比较差异无统计学意义(χ²=0.14,P=0.71>0.05)。1年时间的随访,均未发现复发病例。观察组手术时间、术中出血量、术后住院时间、切口愈合时间均低于对照组,差异均有统计学意义(P<0.05)。观察组术后24 h疼痛评分、肛缘水肿评分均低于对照组,术后尿潴留发生率低于对照组,差异均有统计学意义(P<0.05)。【结论】吻合器直肠后壁痔上黏膜保留切除吻合术治疗混合痔疗效确切,术后恢复快,并发症少。

A Comparative Study of SREMB and Milligan-Morgan on Treatment of Mixed Hemorrhoids

Objective]To investigate the elinical ffe of SREMB(stapled the retum mucosa except the back above hemorrhoidectomy)in the treatment of mixed hemorrhoids.Methods]The elinical data of 90 patients with mixed hem-orthoids treated from January 2017 to December 2017 were analyzed retrospectively,45 cases underwent SREMBC stapled the rectum mucosa except the back above hemorrhoidectomy group),and 45 underwent Milligan-Morgan hemorrhoidecto-my(Milligan-Morgan hemorrhoidectomy group)。The operation,clinical efct and postoperative recovery of the two groups were compared.Results]In the observation group.42 cases were cured and 3 cases were fftive.41 cases were cured and 5 cases were ffetive in the control group.There was no significant diference between the two groups(χ²=0.14,P=0.71>0.05).No recurrent cases were found in l-year fllow up.The operation time.intraoperative bleeding volume,postoperative hospitalization time and incision healing time in the observation group were lower than those in the control group,the difference was statistically signifiant(P<0.05).The pain score and anal border edema score of the observation group 24 h after operation were lower than those of the control group,and the incidence of postoperative uri-nary retention was lower than that of the control group(P<0.05).Conclusion]The efect of SREMB(stapled the ree-tum mucosa except the back above hermorboidectomy)is accurat,the recovery is fast and the complications are few.
