实时荧光定量PCR技术诊断肺结核的meta分析

摘    要:
目的系统评价实时荧光定量PCR技术对肺结核的诊断价值并作meta分析。方法检索Pubmed,EMBASE,BIOSIS,Web of Science以及维普、万方、中国知网、中国医学生物文献数据库。按照Cochrane协作网推荐的诊断试验纳入标准筛选文献,纳入的文献采用QUADAS进行质量评价,用MetaDisc 1.4进行meta分析。结果符合标准的共有21个研究,各研究间存在异质性,但不存在阈值效应。合并后的诊断优势比为105.32,敏感度为0.64,特异度为0.96,阳性似然比为19.79,阴性似然比为0.31。SROC曲线的AUC面积为0.9816,Q*指数为0.9402。结论荧光定量PCR技术检测结核分枝杆菌有着较高的敏感性和很强的特异性,可作为临床肺结核诊断的重要辅助手段,但并不能完全取代痰涂片检查。
