外伤性睫状体脱离早期手术治疗的临床观察

目的 探讨挫伤性睫状体脱离早期行手术治疗的有效性.方法 严重眼外伤患者20例,超声生物显微镜(ultrasound biomicroscopy,UBM)探查外伤眼全周睫状体脱离情况,离断口的范围在45度至135度.术前视力光感～0.5,平均眼压5.6～8.0mmHg(1mmHg=0.133kPa).伴随症状包括外伤性瞳孔散大、虹膜根部离断、黄斑水肿等.所有患眼依据UBM检查结果行睫状体复位术.结果 术后UBM显示所有患眼睫状体复位.术后1个月视力、眼压、前房深度和黄斑部水肿程度均较术前明显好转.结论 早期手术复位睫状体是治疗挫伤性睫状体脱离的有效方法,并可保存有效的视功能.

Clinical observation of direct cyclopexy for traumatic cyclodialysis

OBJECTIVE To observe the clinical characteristics and surgical results of traumatic cyclodialysis.METHODS Twenty cases of complicated ocular trauma were subjected to ultrasound biomicroscopy(UBM) for determining ciliary body detachment,which was 360 degrees associated with the cyclodialysis of 45°~135°.Preoperative visual acuity(VA) ranged from light perception(LP) to 0.5.The average intraocular pressures(IOP) were 5.6~8.0 mmHg before surgeries.Also mydriasis,detachment of the iris root,and macular edema were found.Repair of the cyclodialysis cleft was performed on all patients.RESULTS The ciliary body reattached in all cases.Postoperative visual acuity,mean intraocular pressure,anterior chamber depth and macular edema were significantly improved.CONCLUSIONS For ocular trauma with the cyclodialysis,repair of the cyclodialysis cleft was effective to recover the anatomic structure and preserve the available visual function.
