彩色多普勒超声引导下麦默通微创切除乳腺癌的临床价值

目的 探讨超声引导下麦默通微创旋切术在乳腺癌治疗中的应用效果.方法 选择80例女性乳腺癌患者作为研究对象,根据手术方法的不同分为观察组40例与对照组40例.对照组给予乳腺癌标准根治术,观察组给予超声引导下彩色多普勒超声引导下麦默通微创切除术,记录2组近远期预后情况.结果 所有患者都完成手术,术中无严重并发症发生.观察组的术中出血量与术后住院时间明显少于对照组(P<0.05),但2组手术时间无明显差异(P>0.05).观察组术后1个月的血肿、感染、皮瓣积液、皮瓣坏死、上肢淋巴水肿等并发症总发生率为7.5%,对照组为27.5%,观察组术后并发症发生率明显低于对照组(P<0.05).所有患者随访6个月,观察组的局部复发率、腋淋巴结复发率和远处转移率分别为5.0%、0.0%和7.5%,对照组分别为5.0%、2.5%和10.0%,组间对比差异无统计学意义(P>0.05).结论 彩色多普勒超声引导下麦默通微创切除术在乳腺癌治疗中的应用有更好的微创性,能促进患者的康复,减少术后并发症的发生,且不会增加术后复发率与远处转移率,有很好的应用价值.

Application of Color Doppler Ultrasound Guided Mammotome Minimally Invasive Surgery in the Treatment of Breast Cancer

Objective To investigate the application effects of color doppler ultrasound guided mammotome minimally invasive surgery in the treatment of breast cancer. Methods 80 patient with breast cancer were selected and divided into the ob-servation group and the control group,each with 40 cases . The control group received radical mastectomy,the observation group received color doppler ultrasound guided mammotome minimally invasive biopsy resection,the short-term and long-term prognosis were recorded in the 2 groups. Results All patients were completed surgery and there were no serious complications occurred, the intraoperative blood loss and postoperative hospitalization in the observation group were significantly less than the control group (P<0. 05),and there were no significant difference between the 2 groups in operation time (P>0. 05). The postoperative 1 month,infection,hematoma,seroma,flap necrosis,lymphedema and complication rate in the observation group was 7. 5%,in the control group was 27. 5%,the complication rate in the observation group was significantly lower than the control group(P <0. 05). All patients were followed up for 6 months,the local recurrence rate,axillary recurrence rate and distant metastasis rate in the observation group were 5. 0%,0. 0% and 7. 5%,in the control group were 5. 0%,2. 5% and 10. 0%,there had no statistical difference (P>0. 05). Conclusion Color doppler ultrasound guided mammotome minimally invasive surgery in the treatment of breast cancer has better minimally invasive effect,it can promote the rehabilitation of patients,reduce the incidence of postopera-tive complications,and does not improve the postoperative recurrence rate and distant metastasis rate,it has good application val-ue.
